Responsibilities:

The Diabetes Nurse will:

1. Deliver high-quality care to improve patient outcomes in line with Diabetes guidelines
2. Identify and optimise care for patients with sub-optimally controlled Type 2 diabetes
3. Conduct point of care testing and refer for specialist insulin treatment as needed
4. Provide education, support, and advice to patients with Type 2 Diabetes
5. Fulfil corporate nursing responsibilities and contribute to a proactive care approach

Minimum Requirements:

The ideal Diabetes Nurse will have:

1. A Diabetes diploma or equivalent EN qualification
2. At least 3 years of post-registration experience managing diabetes in primary care
3. Strong knowledge and confidence in providing education and advice to Type 2 Diabetes patients
4. Skills in optimising oral hypoglycaemics
5. A full driving licence and current NMC registration
6. IT literacy, including primary care systems
