We are looking for an outstanding individual to lead our work across the Merton Health and Care Partnership. The postholder will develop and maintain effective partnerships at the most senior level between statutory and non-statutory agencies involved in promoting health and well-being within the borough of Merton. This will involve oversight of the development and delivery of the Merton Health and Care Plan using a Programme approach and specifically how the local health system addresses health inequalities. The post is hosted by South West London ICB but is a joint appointment with London Borough of Merton.
Merton Health and Care Together (MHCT) is the borough based 'Place Committee' for Merton with senior representation from across the local health and care system. MHCT has oversight of the development and delivery of the borough Health and Care Plan. NHS South West London Integrated Care Board (ICB), as part of South West London Integrated Care System (ICS), is a partnership of organisations that come together to plan and deliver joined up health and care services to improve the lives of people in our six boroughs: Croydon, Merton, Kingston, Richmond, Sutton and Wandsworth.
Each ICS consists of two statutory elements:

Duties and Responsibilities
Lead the Programme to deliver the Merton Health and Care Plan and the work of the borough Committee (Merton Health and Care Together MHCT) to improve health within the borough and address health inequalities by:
The postholder will maintain and develop strong working relationships with all members of Merton Health and Care Together (MHCT - the Merton Place Committee), a sub-committee of the Integrated Care Board) and the organisations they represent, across statutory and voluntary sectors. This will involve working across organisations at Chief Officer and Director level.
SW London-wide functions and teams and other borough teams within the ICB.
Provide and receive highly complex, sensitive and contentious information, including presenting information about projects and dependencies to a wide range of internal and external stakeholders in formal settings across Merton.
This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
Salary: £93,903 to £107,412 a year Per Annum inclusive of Outer HCAS
