Job Title: CPN Band 6

Location: Chichester area

Duration: 3-month ongoing contract

Schedule: Monday to Friday, 9:00 AM - 5:00 PM

Start Date: ASAP

End Date: TBC

Rate of Pay: £25.50 - £26.50 per hour

A Day in the Life of a CPN:

Join our team as a Community Psychiatric Nurse and make a real difference in the lives of older adults within a Community Mental Health Team. Your core responsibilities include:

1. Conducting comprehensive mental health assessments and risk evaluations
2. Developing and implementing personalised care and treatment plans
3. Managing a caseload as a care coordinator for service users aged 65+
4. Medication management and participation in duty rotas
5. Working closely with multidisciplinary teams, including psychiatrists, social workers, and GPs
6. Supporting families and carers with education and advice
7. Maintaining accurate and detailed clinical records in line with compliance standards

Requirements & Qualifications:

1. Current and valid NMC registration and PIN
2. Essential: Experience in care coordination and working with older adults
3. Strong communication and interpersonal skills
4. Ability to work both independently and as part of a multidisciplinary team
5. Commitment to delivering high-quality mental health & nursing care
6. Possibility for some hybrid working

Working Hours & Contract Duration:

This full-time role requires availability from Monday to Friday, 9:00 AM - 5:00 PM. The contract is for 3 months ongoing, with a strong likelihood of extension based on performance and service needs.
