Job description

The post holder will be responsible for providing consultant input into the care of people open to East Conwy CMHT.

All referrals to the community Team are managed by a single point of access. There is a meeting on a daily basis. The meeting decides who the most appropriate person in the service is, to see the referred patient. Those taking part in the meeting include senior representatives of the primary and secondary care team. A consultant attends on a daily basis.

The post holder will be expected to comply with the Mental Health (Wales) Measure 2010.

The post may at times attract a junior doctor, depending on availability in the training rota.

Main duties of the job

Post holder will provide specialist psychiatric assessment, treatment and management for adults aged 18 upwards with serious and enduring mental illness in East Conwy. The post holder will provide medical input to patients under secondary care CMHT when required by the care coordinator as well as provide consultation and supervision for Primary Mental Health Services in their catchment area.

The service will deliver a needs led service and therefore the post holder will be expected to manage those above 65 until such time as their needs are more appropriately met by the Older Persons Mental Health Services. (Transition arrangements are under review).

The post holder will be based with the CMHT and work with the county manager to ensure clinical leadership of the multi-disciplinary team. The post holder will assist with clinical supervision of the team. It is anticipated that the post holder will assist in training of team members in clinical matters.

The service operates an Acute Care Model with two dedicated inpatient consultants managing the new to service admitted patients. The inpatient consultants also have responsibility for the Home treatment team patients. The post holder will work collaboratively with the Acute Care Services. There is a dedicated session within the job plan for consultation with the Acute Care Service. In the event of one of the acute care consultants being on leave overall responsibility for the inpatients is by the other acute consultant.
