We are seeking a highly skilled and compassionate Complex Care Nurse to join our team in Worcester, UK. As a Complex Care Nurse, you will provide clinical care in the community, primarily in Worcester and Hereford. This is a permanent position, offering the opportunity to make a significant impact on the lives of patients and their families.

Key Responsibilities:

1. Provide high-quality clinical care to patients with complex medical needs
2. Work closely with other healthcare professionals to develop and implement individualized care plans
3. Administer medications and treatments as prescribed by physicians
4. Monitor and assess patient progress, and make necessary adjustments to care plans
5. Educate patients and their families on managing their conditions and promoting overall health and wellness
6. Collaborate with community resources to ensure patients have access to necessary support services
7. Maintain accurate and up-to-date patient records
8. Communicate effectively with patients, families, and healthcare team members

Requirements:

1. Registered Nurse (RN) license in the UK
2. Minimum of 2 years of experience in complex care nursing
3. Strong clinical assessment and critical thinking skills
4. Excellent communication and interpersonal skills
5. Ability to work independently and as part of a multidisciplinary team
6. Proficiency in using electronic medical records
7. Valid driver's license and access to a reliable vehicle
8. Flexibility to work on a rotating schedule, including evenings and weekends
