This field based role is covering their Milton Keynes catchment area where the company has secured further packages during an exciting growth phase.

The successful CCM will need to have a genuine passion for delivering high standards of care and be committed to really making a difference to the lives of the people they support. Reporting to the Operations Manager, this is a varied role which involves direct communication with patients, relatives, ICB’s and wider care teams.

To work as a Complex Care Manager, you must...

1. Hold an NVQ Level 3 (or equivalent) in Health and Social Care or a related topic
2. Have a proven track record in managing and coordinating care for clients in a community setting
3. Experience in developing and implementing care plans
4. Possess a supportive management style that can bring out the best in others
5. Have advanced communication skills verbally and written
6. Have a full UK driving licence for client visits

The key responsibilities include...

1. Thorough assessments and implementing personalised care plans
2. Inducting support workers and supervising your team members
3. Spot checks and risk management
4. Attending meetings with ICB’s and clients
5. Providing regular reports and updates
