NNICB is seeking to recruit an enthusiastic and forward-thinking nurse (RMN) to work as part of our Continuing Health Care team. The successful candidate will need to demonstrate knowledge, skills, and experience of person-centred assessments and of working within a multi-agency environment. If you are committed to the delivery of high-quality person-centred services, a good leader, and have excellent communication skills, this would be an ideal opportunity for you.
Experience of carrying out Mental Capacity Assessments and Best Interest Meetings is advantageous, and an understanding of DOLS and the Court of Protection would be beneficial to the role. You will need to be confident in using Microsoft Word, Excel, and Outlook and also be confident in learning new systems for recording and report writing.
The successful applicant will be required to work both autonomously and collaboratively in a team and be able to adapt to a rapidly changing environment. The Continuing Care Case Manager will be expected to work within the Continuing Care team to ensure that identified complex cases are managed appropriately using expert knowledge and autonomous decision-making skills.
Working in partnership with patients, carers, GPs, case agencies, consultants, other health professionals, and social care as appropriate, you will be expected to assess and review complex care packages to ensure they continue to meet ongoing clinical needs.
You will lead and participate in Multidisciplinary Team meetings to ensure adequate outcomes of patient care packages and provide specialist clinical advice to the Triage panel for all patients accessing the Continuing Care service.
The role will include case management of adults assessed eligible for Continuing Care.
You will engage in clinical supervision and provide professional support and advice to colleagues and senior managers within the organisation.
