Community Twilight Nurse - Surrey Downs H&C ***£2,000 Welcome Payment

To work within the team, contributing to team management and the development of innovative practice. To be responsible for the assessment of care needs and the development, implementation and evaluation of holistic care packages.

As a Community Staff Nurse - Twilight, you will work within a dedicated team to assess, develop, implement, and evaluate comprehensive care packages for patients, respecting their privacy and dignity. Key responsibilities include collaborating with the District Nurse to lead team initiatives, promoting patient health and well-being, and educating and supporting patients' families and carers. You will maintain effective communication, clear documentation, and high standards of evidence-based nursing care. Additionally, you will mentor team members, participate in clinical supervision, and uphold professional integrity in line with NMC guidelines, while also supporting SDHC's Clinical Governance and Risk Management strategies. Flexibility in working within different SDHC localities as needed is also essential.

Main duties of the job
  • Work as part of the team undertaking lead responsibilities as negotiated and agreed with District Nurse
  • Treat all patients as individuals, respecting their privacy and dignity at all times
  • Assess individual care needs, implement and evaluate holistic programmes of care
  • Involve, support, inform and educate family/carers
  • Promote health and wellbeing of the patient
  • Develop effective communication and liaison systems
  • Maintain clear and comprehensive, signed and contemporaneous records according to SDHC procedures
  • Facilitate/undertake training and professional updating for team and students
  • Act as an assessor and mentor to qualified and unqualified staff
  • Maintain, develop and monitor standards of evidence-based nursing care
  • Maintain own professional and clinical integrity in line with NMC guidelines
  • Support/mentor team members and participate in clinical supervision activities
  • Work without direct supervision and maintain close liaison with the senior team member, deputising as required
  • Be responsible for maintaining timely patient care
  • Be responsible for maintaining data entry and other record keeping
  • Work in cooperation and partnership with all other professionals and agencies involved in the care of the patient
