Main duties of the jobOur care community teams is based in The Fountains Health Center covering the surrounding geographic area. It's important that you can drive and have access to your own vehicle for work. But of course, mileage expenses are reimbursed in addition to your hourly rate.
As a Community Staff Nurse the successful candidate will undertake duties involving the delivery of high-quality nursing services and the holistic assessments of patients ensuring a person-centred approach.
You will work alongside various professionals to provide quality and seamless care to patients predominantly in their own home. You will be proactive in coordinating their care needs as appropriate, avoiding unnecessary admission to hospital or long-term care settings.
