The post holder will join a team of skilled and patient-focused clinical pharmacists and medicines management technicians based in Tiverton Hospital who provide care to inpatients and patients in their own home. You will work with colleagues to enable the safe and effective use of medicines through provision of medicines optimisation and personalised medication reviews.
