You are a Band 6 Community Psychiatric Nurse working in a fast-paced Home Treatment Team based in Kensington and Chelsea - the post offers a myriad of advantages. Firstly, it's a hub of cultural diversity, providing exposure to different perspectives and fostering a vibrant atmosphere. The area is full of must-sees such as Portobello Market, visiting the Japanese Gardens in Holland Park, and home to major museums of South Kensington.

Main duties of the job

This role involves intensive support and treatment to patients in the community in the acute phase of a mental health crisis as an alternative to inpatient care; monitoring patients' safety and well-being to promote independence in the community; acting as gatekeepers to admissions; supporting early discharges from hospital.

You will also be expected to work in collaboration with the Team Manager and Clinical Team Leaders/Senior Practitioners and to have a close relationship with the community teams and inpatient services.

Job responsibilities

Skills and Abilities:

Effective communication skills both verbally and through written reports.

Ability to assess complex risk and produce treatment plans to manage this; ability to manage complex decision-making situations.

Effective leadership skills; ability to supervise junior staff and students; teaching and assessing skills.

Ability to reflect and critically appraise own performance.

Ability to work effectively in stressful and emotionally charged situations; able to manage personal stress.

Clinical duties:

To provide a comprehensive mental health assessment for complex patients in the acute phase of a mental health crisis.

To assess current risks and identify how the risk and patients' mental health needs would be best met.

To formulate, implement, and review treatment plans with patients and their carers that aim to address their psychological, physical, and social needs.

Person Specification

  • Experience in community mental health.
  • Experience in crisis mental health work.
  • QI experience.

Previous Experience

  • Extensive post-registration experience in an acute adult setting or community setting.
  • Previous experience of working within a HTT.

Education and Qualifications

  • Current NMC Professional Registration RMN.
  • Evidence of ongoing professional development.
  • Full UK driving licence.

Skills and Knowledge

  • Good interpersonal and communication skills, both verbally and written.
  • The ability to assess, develop, implement, and evaluate programmes of care for individuals experiencing acute mental distress.
  • Ability to carry out risk assessments and implementation of appropriate risk management plans.
  • Have a holistic understanding of mental health conditions and their impact on individuals' psychological, psychosocial, and physical health.
  • Knowledge of the Mental Health Act and other relevant legislation.
  • A commitment and the ability to work as part of a team.
  • Knowledge of issues and developments within Mental Health and the National Health Service.
