Job description

Band 6 Community Physiotherapist - Croydon Health Services. The successful candidate will deliver physiotherapy services in the community, supporting patients in their own homes or care settings as part of a multi-disciplinary team.


This is a full-time role for an initial 6-month period with potential for extension, subject to service demand and performance.


Key Responsibilities:
  1. Provide physiotherapy assessments and treatment to patients in the community setting
  2. Work autonomously and as part of a wider multi-disciplinary team
  3. Maintain accurate and up-to-date patient records in line with NHS standards
  4. Plan and implement individual treatment programmes for patients with a variety of conditions
  5. Contribute to discharge planning and onward referral where appropriate
  6. Liaise with other healthcare professionals, patients, carers, and families to ensure continuity of care
  7. Travel between patient homes and community bases as required
Essential Requirements:
  1. Previous experience in a community physiotherapy role
  2. Valid HCPC registration as a Physiotherapist
  3. UK clinical experience at Band 6 level or equivalent
  4. Car owner and willing to drive for work purposes
  5. Able to work under the Direct Engagement model
