Job description

Job Summary

We are looking to recruit an experienced senior nurse to work as a Band 7 Community Neurology Specialist Nurse with interest in Parkinson's Disease/MS in a joint role working across Essex Partnership University NHS Trust and Princess Alexandra NHS Trust.

We welcome applications from experienced or existing Band 7 Parkinson's Disease and MS Specialist Nurses as well as nurses working at Band 6 level aspiring to progress into a specialist nursing role within neurology.

The post holder will develop, implement and evaluate a seamless community PD/MS specialist service ensuring that patients receive the highest standard of clinical care and that specialist advice, education and support to health professionals and patients is provided. The post holder will drive service development, audits and research within the Community Neuro service in conjunction with Princess Alexandra NHS Trust.

They will work collaboratively with the established Primary Care Networks, ensuring cohesive leadership across the three localities.

The post holder will be an integral part of the West Essex Care Closer to Home model to meet the needs of the local population. In addition, they will work closely with the Virtual Hospital and Care Co-ordination Centre (CCC) to prevent hospital admission, facilitate hospital discharges and ensure care is delivered in the right place at the right time.

Main Duties of the Job

  1. Work jointly with Princess Alexandra NHS Trust Neurology Team to contribute to a seamless service for patients.
  2. Function autonomously in an adept manner to assess, manage, plan and implement treatment tailored to individual patients across the Parkinson's disease spectrum in both routine and non-routine circumstances.
  3. Demonstrate an equitable service through audit and clinical effectiveness.
  4. Maintain the Parkinson's Disease/MS patient caseload including acceptance of appropriate referrals, triage, waiting times and discharging patients to a dormant caseload when active input is no longer required.
  5. To develop, deliver and evaluate a plan of care relating to symptom management, which will include assessment, planning, implementing, reviewing and documenting care plans in agreement with the person with Parkinson's Disease/MS. This will include but not exhaustive of ordering diagnostic tests and making referrals.
  6. Support and facilitate patients to self-manage their disease ensuring they know when to seek help.
  7. Actively participate in service development.
  8. Active involvement in a multidisciplinary approach to the decision making process regarding commencing disease modifying treatment. Participate in ongoing delivery of treatment and monitoring of patients while on treatment.
  9. To actively involve service users in providing feedback of their experience of the current service and suggestions for improvements.

Person Specification

Education/Qualifications Essential:

  • At least 3 years post registration experience working at Band 6
  • Non-medical Prescriber or willingness to undertake within the first year in post
  • Relevant post registration training in Parkinson's Disease/MS or willingness to undertake

Experience Essential:

  • Extensive understanding of palliative care needs, EOL care planning and providing support
  • Ability to communicate complex information to patients and relatives who may be anxious
  • Ability to provide and receive highly complex, sensitive and potentially distressing information

Corporate Essential:

  • Current driving licence
