An established industry player is seeking a Community Mental Health Practitioner to join their dedicated team. The Mental Health Practitioners have a crucial role in processing, assessing and determining the mental health and social care needs of a person referred to determine their ongoing needs and facilitation of service provision. The role will require excellent risk assessment and management skills in relation to and positive risk taking and decision making to enable safe and effective responses for an identified client group, largely by telephone/assessment.
You will participate in the delivery of the agreed aims and objectives of the service and identify factors which may inhibit these from being achieved. They will assist in the delivery of a high standard of Mental Health care to service users referred to the service. They will implement agreed systems for the monitoring of effectiveness of care.
You will be based with either the Shropshire or Telford Crisis Resolution Home Treatment Team (dependant on preference). At times, there will be some expectation to be based at either The Redwoods Centre or Hall Court.
You will be working closely with the Crisis team and the Access Call handlers.
The post is working 22.5 hours per week and will alternate each week between:
Week one: Monday to Friday 4pm - 8.45pm
Week two: Saturday and Sunday 8am - 8pm
