Job description

This is an exciting time to join the community service as we continue to grow and work as a seamless multidisciplinary team, so we can be there to support more people at end of life. Our services work across 7 days a week delivering a mixture of direct planned and responsive care and supporting other professionals through our expertise and experience. Above everything else, our service is patient centred.

The role is predominantly office based and will provide comprehensive administrative and clinical support to the CPCT Hub 5 days a week. It may also include deployment to support outpatient clinics, and urgent community visits when required, and provide ongoing support for the stable patient caseload.

Main duties of the job

The post holder will work without direct supervision to facilitate and coordinate daily patient flow within the Community Palliative Care Team (CPCT) Hub. Utilising their own initiative, the postholder will proactively identify opportunities and actions to support the clinical team in the Hub, patients, and families.

St Barnabas House provides specialist palliative care to adults in the Worthing, Adur, Arun and Henfield areas through a range of services, including in-patient care and community living well services.

Job responsibilities
  1. Communicate effectively with patients, carers and colleagues, both internally and externally, maintaining positive working relationships.
  2. Ensure that patients are always treated safely and courteously.
  3. Use empathetic and effective communication skills for a wide range of patients and their carers, particularly where information may be sensitive.
  4. Use electronic communication and keep accurate patient records in line with policy and procedures.
  5. Liaise with external healthcare providers, completing referrals as required.
  6. Liaise with internal teams, ensuring that positive patient outcomes are achieved.
  7. Follow through agreed actions to ensure best outcomes for patients including supporting to complete Fast Track Continuing Care applications, referrals to ECHO hub, Blue Badge applications, follow up medication requests to GP surgeries, and any other duty which will ensure effective patient flow.
  8. Work unsupervised, planning and organising tasks, recognising that there may be a need to reprioritise throughout the day.
  9. Appropriate support patient referrals, ensuring that the patient receives the right care by the right team, in a timely manner under the guidance of Duty Clinical Nurse Specialist (CNS).
  10. Make patient transport bookings in conjunction with the Duty CNS/Clinical Admin IPU as requested for patients to minimise delays.
  11. Be the point of contact for incoming enquiries from both internal and external sources, prioritising and actioning as required under the guidance of Duty CNS.
  12. Exercise judgement when dealing with patients, their carers and other healthcare professionals, escalating concerns appropriately.
  13. Identify and escalate concerns regarding patient referrals and report in a timely manner to the Duty CNS, including acting to ensure patient safety and follow up as required with other services under the direction of the Duty CNS.
  14. Support with urgent same day community visits as identified by the Duty CNS and as required.
  15. Support with ongoing contact for stable patient caseload, within an identified time frame.
  16. Under supervision, support the Head of Clinical Services with administrative duties to include completion of Community Palliative Care Team rota, and coordination of team annual leave to ensure safe staffing levels.
Person Specification
Experience
  • Good communication skills, with a calm and professional manner.
  • Be able to work methodically and independently, whilst being a good team player.
  • A demonstrable insight into palliative care.
  • Be able to use technology to process and facilitate patient flow within the community teams and ensure there are safe staffing levels.
  • Confidence in coordinating other senior healthcare professionals to facilitate case conference including administration of these.
  • Have awareness of the pressures within the Hub and provide adaptable and flexible support to the senior team, adjusting priorities as appropriate.
  • Prompt identification of areas to provide support within the Hub, including identifying appropriate pharmacies to obtain stock.
  • Organisational skills to procure/manage resources for the Hub.
  • Able to source support for patients/carers from external agencies and follow through to completion.
  • Use initiative when responding to changing patient signs with regards to existing clinical plans.
Qualifications
  • Administrative and coordination skills with relevant Level 2/3.
  • NVQ or equivalent qualification or experience.
