Hounslow and Richmond Community Healthcare NHS Trust – Community Healthcare Support Worker – Band 3
Hounslow and Richmond Community Healthcare (HRCH) NHS Trust are seeking an enthusiastic and qualified Community Healthcare Support Worker to join them on a temporary basis to support their District Nursing Team alongside individuals, families, and the wider community, and other primary healthcare services, e.g. Health visitors, School Nurses, Social Services etc.

HRCH provides community health services for around 523,000 people registered with GPs in the London boroughs of Hounslow and Richmond. The Trust currently employ around 1,300 people, who work across a wide range of health centres, hospitals, GP surgeries, children’s centres, local council facilities and in community settings – including in people’s homes. This helps people across the community to stay well, manage their own health with the right support and avoid stays in hospital.
Responsibilities:
- Implement treatment programmes with individual patients and/or carers in their own homes as directed. Provide high standards of care as delegated, encompassing a rehabilitative approach where necessary, and report back as agreed.
- Carry out clinical duties and be prepared to undertake clinical skills as per needs of the service and specific patient after training and competence assessment with support from an identified mentor (e.g., venepuncture, Blood Pressure, pulse and temperature recording, urinalysis, blood glucose monitoring, simple wound dressings, and non-complex assessments such as continence reviews).
- Undertake basic duties as delegated and overseen by the registered nurse and actively report any changes in a patient’s condition in a timely way to the registered nurse.
- Liaise with other healthcare and multi-agency workers to provide a multi-disciplinary approach to care.
- Communicate with patients at a time and level appropriate to their needs in a manner that promotes positive health.
- Meet at agreed times to discuss patient care and collect allocated worklist from District Nurse Team Leader or Community Staff Nurse, and take referrals from other professions as agreed, reporting back to caseload holder.
- Ensure that equipment and aids necessary for effective care meet the needs of individual patients and are adequately maintained. Demonstrate and explain safe use of equipment for patient and/or carer where required.
- Demonstrate, teach and supervise simple procedures that can be safely delegated to patient/carer with support and approval from the District Nurse Team Leader or Community Staff Nurse, as necessary.
Important Requirements:
- GCSE grade A – C in English or Maths.
- Ability to communicate and build rapport with patients, families, carers, colleagues, and other professionals to progress the care of the patient.
- Experience of working in the NHS.
- Commitment to providing a high-quality, professional, caring service to all patients.
- Excellent administrative, verbal, and written communication skills.
- Experience of working as a team.
