Job description

Are you an experienced RMN or RNLD seeking a challenging and rewarding role in a dynamic mental health setting? Join us as a Community Case Manager (RMN/RNLD) in a well-established provider of adult and paediatric complex care within the Kent community.


As a Community Case Manager (RMN/RNLD), you will oversee community-based mental health and learning disability care packages. This involves working closely with children, adults, and their families to create personalised care plans, while mentoring and training carers to develop their clinical skills. You will also collaborate with commissioners and ICBs to acquire and maintain care packages, ensuring excellent support and care for all clients.


Key Responsibilities for Community Case Manager (RMN/RNLD):
  1. Manage complex care mental health and learning disability packages for both paediatric and adult clients.
  2. Train and mentor regional nurses and HCAs, improving their skills and care delivery.
  3. Work with commissioners and ICBs to acquire new packages and foster strong, ongoing relationships.
  4. Maintain visibility with clients, families, and the clinical team to ensure high-quality care.

Criteria for Community Case Manager (RMN/RNLD):
  1. Registered Mental Health Nurse (RMN) or Registered Learning Disability Nurse (RNLD).
  2. Active UK NMC Registration.
  3. Minimum of 2 years post-qualification experience.
  4. Experience in community mental health or Tier 2 CAMHS.
  5. Valid UK driving license and access to a car.

Benefits:
  1. Up to £50,000 per annum DOE.
  2. 2 days working from home / 3 days in the community.
  3. Monday to Friday (9-5) hours.
