About the Role

The Community Care Officer role is fast-paced and requires flexibility, resilience, and an ability to prioritize and adapt to changing situations. Community Care Officers are required to complete Care Act Assessments face-to-face, while assessing levels of risk, identifying safeguarding concerns, and considering mental capacity issues. This role will specifically be working with citizens who have received a period of reablement support and then require a Care Act Assessment of their long-term needs. They will work with citizens and their networks of support to provide appropriate support that will enable independence, choice, and control.

This vacancy relates to two positions: one full-time position of 37 hours a week and one part-time position of 18.5 hours a week.

About You

The ideal candidate will have:

  1. The ability to undertake adult-focused assessments of need within a person-centred approach.
  2. Experience of working with adults experiencing a range of difficulties relating to learning disability, mental health, substance misuse, and physical health problems.
  3. The ability to manage and prioritize a caseload and work autonomously.
  4. The ability to establish effective working relationships with colleagues within the team and from other agencies.

A DBS enhanced check for a regulated activity is required for this post.

At Nottingham City Council, we believe that work is what you do, not where you do it. We offer different working arrangements, depending on the role, including hybrid working.
