Position: Community Care Lead

Salary: £30,000.00 per annum

Requirements:

1. Minimum 1 year in care coordination is desirable.
2. You will be the heartbeat of our care services, building long-lasting relationships and driving engagement with our care teams and clients.
3. Ideally, you would have worked within complex home care, and you are passionate about high-quality person-centered care.
4. You will have strong people management and supervisory skills and an appetite to support, develop, and nurture individuals within your team.
5. Strong organisational and communication skills.
6. Valid UK Driver's licence and the ability to travel across the Nottingham area is essential, ensuring visibility with our clients and care teams.
7. You will work collaboratively with our support service teams such as recruitment, compliance, and scheduling.

Responsibilities:

1. Ensure service users are at the heart of the care delivery and their needs and preferences are accommodated.
2. Supervise staff to ensure efficient service operations.
3. Organise and coordinate care based on industry standards and policies.
4. Deliver services promptly as per individual agreements and contract standards.
5. Promote and protect the welfare of supported individuals.
6. Foster effective working relationships with fellow employees and multidisciplinary teams.
7. Willing to travel.
