Position: Community Care Responder
Hours: Guaranteed 30 hours per week with shift patterns of 7:00am till 2:30pm and 3:30pm till 10:00pm, 2-week rolling rota with alternate weekends.

Our primary goal is to provide the most appropriate support to individuals and families so they can remain in the comfort and familiar surroundings of their home environment, preserving their independence, dignity, and lifestyle choices after being in the hospital.
This is a new service to the area and a way for us to take the pressure off the NHS by supporting those coming out of the hospital and ensuring they have the best support to enable them to reduce the levels of care they require.
You will be the first responder, trained to a high level, to ensure you can deliver the best possible outcomes to our clients. You will be dealing with various clients, ranging from those who just had surgery to those recovering from broken bones and illnesses, by helping them set and reach their recovery goals.
The role is focused on the client’s recovery and their end goal to lead a happy, healthy, and independent life. There will be aspects of personal care (bathing, washing, toileting, medication) and motivating them daily to meet their objectives and promote their overall well-being. As part of this, you will receive assistance from Occupational Health and Physiotherapists where required.
We have numerous full-time positions available in this role, and you must be available to start immediately following recruitment checks.
