An exciting opportunity has arisen within the General Surgery directorate for a Colorectal PSFU Coordinator to work as part of the General surgery team. We are a strong, proactive team who welcome innovation andadaptability within the service.

The position is part time (25 hours) and the successfulapplicant will be based at Worcester Royal Hospital. We also offer someflexibility with home working for a limited amount of days per week.

The Colorectal Cancer Support Worker role will form an integral part of the multidisciplinary team approach in caring for the physical, social and psychological needs of patients diagnosed with cancer and their carers throughout their care treatment pathway and after care support.

The Support Worker will work in close unison with the Clinical Nurse Specialists to ensure the co-ordination and management of patients at all points of their treatment but in particular the Support Worker will also act as a point of contact for patients who will have been prepared by their clinical team for Patient Centred Follow Up (PCFU) and will be able to sign post patients to support their self-management.

The Support Worker will provide vital administrative support to the Colorectal Cancer Team and in particular the Clinical Nurse Specialists by co-ordinating the PCFU pathway and assisting patients and carers with concerns and queries.

Main duties of the job

Provide information to patients related to treatment/follow-up and signpost to appropriate healthcare providers within remit of role and guidance.

To participate in collating patient surveys/patient experiences surveys and develop action plans alongside the clinical nurse specialist.

Support CNSs in developing and facilitating Health and Well Being Programmes / Beyond Cancer Programme. This will involve contributing and delivering education to patient and carers, including information around health promotion, Colorectal Cancer information and supporting self-management.

Develop good working relationships and liaison between other departments and healthcare professionals within the Trust cancer services.

Communicate directly with patients, relatives and carers demonstrating empathy, understanding and respect. Information discussed and received will often require tact or persuasive skills, to ensure safety and understanding around treatment side effects as well as an understanding of complex information around treatment options.

Collaborate and work closely with local providers of validated information resources, users and user groups, and other registered providers e.g. Macmillan, Cancer Research UK, Support Groups, and wider NHS Trust groups.

Liaise with the MDT co-ordinator to ensure seamless service for patients and smooth transition through the cancer pathway

Demonstrate excellent communication skills both verbal and written.

Job responsibilities

The post holder will work independently within the remit of the role but under the guidance and supervision of the Clinical Nurse Specialists to

Analytical and Judgement skills

Access and input confidential information through integrated databases in accordance with the agreed policy and procedure.

Demonstrate a broad understanding of cancer treatments offered within the Cancer Services.

Be aware of the particular physical and psychological needs of cancer patients, observe and listen and report any patients concerns, alerting the CNS so that any necessary treatment and or reassurance can be given to maintain patient safety and well-being. This will involve prioritising a patients clinical and psychological symptoms and judging how best to action any concerns when there may not be a straightforward choice of options.

Planning and Organisational skills

Plan and organise own workload to ensure patient needs are prioritised. Plan and organise own day to day workload, which will also involve liaising with several other healthcare professionals and external agencies.

Responsibility for Patient and Client care, treatment and Therapy

Monitor and check appointments and diagnostic tests are booked and undertaken in an appropriate and timely manner.

Ensure patients receive timely and appropriate information in regard to their diagnosis and treatment and signpost to appropriate supportive leaflets and literature.

Respond to telephone queries by a model of triage and appropriate referral to CNS or other allied healthcare professional.

Support the delivery of appropriate information to patients in consultation with the multi-disciplinary team and according to any agreed information pathways, including printing of information

Responsibility for Policy and Service Development Implementation

Participate in and contribute to service development

Participate in initiatives to explore patient experience and service feedback

Assist the CNSs with the implementation of Treatment Summary Records

Participate in the development and delivery of Health and Well Being Sessions for Living with and Beyond Cancer.

Support and coordination of remote surveillance for patients on Patient Centred Follow up (PCFU) pathways to ensure risk free survivorship

Communicate effectively with relevant MDTs and clinical teams to ensure that all new and migrated PCFU patients are identified for coordinated support as assessed by health professionals.

Be responsible for maintaining the IT Tracker Somerset database for patients on PCFU, monitoring clinical alerts, organising relevant tests required and ensuring results are flagged to the named clinician and MDT.

Be responsible for the regular monitoring, auditing, and reporting of compliance with the remote surveillance pathways in line with the governance protocols.

Contribute to clinical and IT team reviews of the implementation of remote surveillance systems and any issues identified, contributing to solution identification and resolution.

Ensure all documentation is recorded accurately, legibly signed and dated in accordance with Trust policies.

Act as the patient advocate and facilitator in order to resolve issues that may be perceived as barriers to care.

Responsibility for Human Resources, e.g. Supervision, Training, HR Advice

and Management

Participate in mandatory training and education opportunities for self- development and shared experience.

Support colleagues, demonstrating own activities and providing advice as necessary.

Demonstrate excellent interpersonal and communication skills.

Share knowledge from training days attended and actively participate and contribute to the Practice Development within Cancer Services.

Participate and display willingness to learn, develop a relevant technical competency / area of interest and keep up to date with current national and local issues and developments.

Responsibility for Information Resources and Administrative Duties

Attend MDT meetings, assist with preparation of MDT and provide assistance to the CNS with arranging tests, investigations and outpatient appointments as a result of MDT decisions

Use computer skills to document assessments

Ensure accurate and timely documentation

Coordinate access to patient information and education resources for patients.

Facilitate the delivery of Health and Well-Being sessions to patients and carers, as part of the National Cancer Survivorship Initiative.

Responsibility for Research and Development

To support and contribute to clinical audit and research initiaitives.

Person Specification

Qualifications

  • GCSE or equivalent in English and Maths
  • Experience and skills in using patient administration systems (ie, Oasis, Bluespier) with an in-depth working knowledge of the applications
  • Experience and skills in using the standard range of Microsoft Office programmes, particularly Word and Excel
  • RSA II typing or equivalent level of skills
  • Evidence of continued personal development
  • Willingness to undertake further education and training applicable to the role
  • Experience and skills of using Somerset Cancer Register

Experience

  • Administrative experience, supporting clinical teams and senior managers
  • Previous clerical or secretarial experience within the NHS particularly developing and maintaining office systems
  • Experience of working with complex and sensitive issues on a day to day basis, including contact with patients and relatives who may be discussed.
  • Maintenance of databases/other electronic systems as required
  • Experience of working with cancer services
  • Experience of working within a multi-disciplinary team
  • Experience of working within an NHS Acute Hospital setting
  • Experience of communicating with patients and relatives who are being treated on cancer pathways
  • Experience communicating with patients via telephone

Knowledge and Skills

  • Understanding of medical terminology
