In our quest to deliver patient-focused quality care, we are looking to recruit 2 highly motivated, innovative, and enthusiastic Clinical Team Leaders to join our forward-thinking, friendly, and expanding Health & Justice team in the Kensington, Chelsea & Westminster ReFocus Team. By joining our Community Forensic Mental Health Team, you will not only look after the healthcare needs of our patients residing in secure environments, but you will also have the opportunity to have a real impact on promoting health and well-being. Our healthcare services work to a 5-day week working model to ensure our patient's healthcare is managed and maintained to the highest of standards at all times.

Our motto is “Caring NOT Judging” so by working in partnership with the criminal justice system and other agencies, we provide responsive, dependable, and high-quality primary care and mental health services to those who may have offended and those at risk of offending in the communities we serve, to improve their health status and to encourage social inclusion.

Responsibilities:

  • The post holder will work as a Clinical Team Leader (Occupational Therapist/ Social Worker/ Registered Mental Health Nurse) in the Kensington, Chelsea and Westminster Forensic Community Team providing brief clinical and social care intervention for service users.
  • To triage referrals and co-ordinate duty assessments.
  • Be responsible for the supervision of staff & their case load.
  • Facilitate appropriate clinical practice of the team including CPN’s, OT’s, Administrative staff, and AMHPS.
  • Draw up the monthly rotas.
  • Develop and maintain close professional relationships with GPs, Community Mental Health Hubs, Specialist Community Forensic Services, and other professionals.
  • Ensure staff practice in a safe, appropriate, and professional way with service users and their carers.
  • Attend professional practice forums.
  • Organise and investigate Safeguarding alerts in line with local Safeguarding policies.
  • Chair clinical and team meetings as appropriate.
  • Provide senior support to the Team Manager.
  • To assist the team to be compliant with KPI and monitor electronic data.
  • To take a lead role in Dual Diagnosis within the Team.

Clinical Practice:

  • The post holder will work in partnership with other teams and provider agencies to refer service users who need long-term support as necessary.
  • Attend regular management and senior team meetings as appropriate and to deputise as necessary.
  • Ensure staff keep accurate and contemporaneous electronic notes and input data as required.
  • To undertake holistic assessment of the mental health needs of adults in the community.
  • To identify vulnerable service users in Primary Care and deploy resources in an effective manner to meet their needs within existing budgets.
  • To carry out needs assessment of individuals presenting to the duty service using FACS criteria.
  • To promote and support self-care, recognising the important role of individuals, their families, and carers as co-producers of health.
  • To identify vulnerable adults and provide effective programmes of support that protect and promote their health and wellbeing, intervening appropriately when a person’s safety or welfare is at risk.
  • To ensure that CNWL Trust and LA protocols are adhered to by team members.
  • To cover for the absences of senior colleagues as appropriate and as required by service needs.
  • To contribute to the implementation of the Clinical Governance agenda including Target Setting for Teams, Clinical Audit.
  • Participate as a key member of the Borough Service to ensure collaborative and innovative practice.
