Job Summary

Is it time your skills were recognised? At NSFT South Adult Community Services, based at Wymondham, we have an exciting opportunity for an enthusiastic, motivated individual to become part of the team and support service users and carers affected by mental health.

We currently have two substantive Band 3 Clinical Support Worker positions working in the community. Our team works together to support each other while giving you the autonomy to work with individuals, develop a therapeutic relationship, and support individuals to live as independently and fulfilling life as possible.

We are a recovery-focused, multi-disciplinary team providing an effective, supportive, and professional service for our Services users and Carers. We want to build on and continue to improve services so we can be the safest trust with gold standard practices. We are an open and transparent trust and want to learn from past mistakes to provide the best service possible.

Main Duties of the Job

You will undertake the planning, with the support of a registered professional, and delivery of direct patient care for your caseload, enabling service users requiring secondary mental health services to develop independence in the community and access the right resources to support their recovery. This will be with supervision, ensuring continuity of practice and working within a multi-disciplinary and multi-agency environment.

Job Responsibilities
  • Undertake a range of delegated clinical health duties in relation to patient care in a hospital, community, or other setting, under the direct supervision of a Registered Practitioner or Assistant Practitioner.
  • Undertake basic home visits and report back to Registered Practitioners after face-to-face contact with service users.
  • Undertake medicine drops to service users.
  • Engage in joint working with Registered Practitioners, e.g., support practitioners on assessments where a lone visit would not be appropriate.
  • Support service users in participating in daily activities within the community, e.g., banking, shopping.
  • Undertake home visits approved and risk assessed by the Registered Practitioner, subject to regular review through regular supervision.
Person Specification
Experience

Essential:

  • Previous experience of working within a mental health setting.
  • Care Certificate.

Desirable:

  • Previous experience of working within a CMHT.
  • NVQ L2/3 in H&SC or Diploma L2/3 in H&SC or apprenticeship L2/3 in H&SC.
  • VRQ L3 in mental health.
Skills

Essential:

  • Demonstrates ability to work independently.
  • Able to use own initiative while liaising with other team members and a good level of self-awareness and interest in self-development.
  • Respectful of service users' individual needs and requirements within their own environment.
  • Good observation and reporting skills.

Desirable:

  • Experience of working with the Recovery model.
Knowledge

Essential:

  • Understanding of issues surrounding confidentiality.
  • Well-motivated with an interest in mental health issues.
  • Able to travel independently.
