The Urgent Community Response Team is looking to recruit two new CSWs into our service (one part-time and one full-time). As part of UCR, you will be involved in directly delivering patient care to individuals living in their own home.
The Urgent Community Response team is responsible for providing a 2-hour response service to prevent unnecessary admissions. You will work collaboratively with colleagues and other services to support patients in remaining as independent as possible in their own home.
Day to day, you will support Clinical Assessors and Assistant Practitioners in responding to new referrals. You will also assist with follow-up visits and provide care in the community.
We require a caring and compassionate support worker to join part of our new service in the south of Nottinghamshire. Posts are available at Parkhouse Health and Social Care Centre.
Urgent Community Response is designed to help patients stay out of the hospital and uses the latest systems and technology to provide the most efficient and effective healthcare possible via a 2-hour response to patients experiencing a sudden deterioration in their health and wellbeing.
The team enables individuals to increase independence and improve health outcomes by designing programs for recovery and self-care management through timely assessments, interventions, rehabilitation, and management of long-term and life-limiting conditions. You will work in collaboration with the patient, their carer, and other statutory and voluntary organisations/agencies, playing a vital role in transforming community health services and ensuring high-quality care is provided based on the needs of our patients.
The service operates 7 days a week from 08.00 - 22.00hrs.
We are looking to recruit motivated, caring, compassionate, and experienced Community Rehab Support Workers to join this new service within the South of Nottingham, covering the localities of Rushcliffe, Broxtowe, and Gedling. You will work as an accountable and autonomous practitioner alongside integrated care teams and acute Trusts to support discharge and prevent hospital admission.

You must have a full UK driving licence and vehicle for business use for this post; however, reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.
This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975, and a submission for Disclosure to the Disclosure and Barring Service (formerly known as CRB) will be necessary to check for any previous criminal convictions.
