The successful applicant will be supported by a high-functioning Partnership and experienced clinical and administrative team, including a wider pharmacist team.

About the role:

• Joining a team of two prescribing pharmacists, 3 pharmacy technicians, 1 medicines management coordinator, 1 care coordinator.
• Main priority of the role will be to support with medicines-related activity such as completing medication reviews, carrying out QOF work, medicines optimisation, CQC audits, Eclipse.
• Supported by a PCN Manager and Clinical Director, Prescribing Lead GP Partner, and wider clinical workforce.

Main duties of the job

The Key Responsibilities for the role will be:

• Complete medication reviews as assigned.
• Carry out medicines optimisation workstreams such as STOMP.
• Complete QOF activities such as medication commencement and titration.
• Complete designated CQC audits in a timely fashion.
• Support with the safe review of MHRA and CAS alerts and ensure dissemination and learning.
• Oversee the safe prescribing of high risk monitoring in line with LMSG and ensure regular audit and robust safety processes.
• EPS - supporting GPs to sign EPS as needed.
• Supporting with medication queries and reviews as well as managing the flow of work through the system.
• Lipid management.
• PCN DES Extended hours - to provide some extended hours as required.
• Green agenda - reviewing inhalers and recycling possibilities and other aspects of the green agenda for general practice as related to meds.
• Scripts Workflow Support the scripts desk in any acute queries on the day and assist GPs in signing of EPS scripts.
• Honorary contracts with the Market Harborough Pharmacy and to support in services i.e. Hypertension, Pharmacy 1st, NMS, contraceptive reviews.

Market Harborough Medical Centre will be the main location of the job and is situated within walking distance of the town centre. We are a single partnership PCN operating out of three sites: Market Harborough Medical Centre, Husbands Bosworth Surgery, and St Luke's Treatment Centre. We have 34,000 patients and 10 care homes. Job responsibilities

Key Objectives:

• Provide pro-active leadership on medicines and prescribing systems to the wider practice team, patients, and their carers.
• Develop and deliver patient facing services and clinics which improve patient outcomes, reflect the needs of the practice population and shift workload from other disciplines.
• Deliver high quality medication reviews and develop bespoke medicines treatment plans for patients (particularly the elderly, complex patients, and those at high risk of admission).
• Improve the quality and effectiveness of prescribing through clinical audit and education to improve performance against NICE standards, prescribing guidance, and other clinical standards.
• Develop yourself and the role through participation in clinical supervision, training, and service redesign activities.
• Deliver training, mentoring and guidance to other clinicians and staff on medicines issues.

Patient Facing Clinical Services & Care:

• Work in a patient facing role to clinically assess and treat patients using their expert knowledge of medicines for specific disease areas.
• Take responsibility for the care management of patients with chronic diseases and undertake clinical medication reviews to proactively manage people with complex polypharmacy, especially the elderly, people in care homes, those with multiple long-term conditions (in particular COPD and asthma) and people with learning disabilities or autism (through STOMP - Stopping Over Medication Programme).
• Prescribe acute and repeat medication for patient need within areas of competency.
• Provide person-centred medicines optimisation. Through structured medication reviews, support patients to take their medications to get the best from them, reduce waste and promote self-care.
• Support the system of reconciliation of medicines from outpatient and discharge letters and support safe transfer of care through liaison with hospital and community colleagues.
• Take an active role in the clinical aspects of shared care protocols, clinical research with medicines, liaison with specialist pharmacists (including mental health and reduction of inappropriate antipsychotic use in people with learning difficulties) and anticoagulation.

Safety:

• Report medicines related incidents, contribute to investigations and root cause analyses.
• Participate in serious incident investigations and multidisciplinary case reviews.
• Report Adverse Drug Reactions and promote reporting by MDT.
• Action and respond to Drug Safety Alerts and other safety cascades.

Training, Education & Information:

• Develop and deliver education sessions and training to clinical and non-clinical staff on medicines issues.
• Provide ad-hoc medicines advice to doctors and other clinicians.
• Assess emergent clinical evidence and develop strategies for implementation in practice.
• Horizon scan for new drugs and provide information to the MDT on their introduction.
• Provide advice on cost-effective prescribing and supply issues.
• Provide teaching and support to medical students, GP trainees and other trainee healthcare professionals.
• Always seek to develop own knowledge and understanding.

Quality:

• Provide leadership on medicines safety compliance with CQC and other professional standards.
• Promote a culture of constant improvement and excellence.
• Conduct regular clinical audits and other improvement activities.
• Assess own performance regularly and make suggestions of how to improve personal and team performance.

Health & Safety:

• Assist in promoting and maintaining your own and others health, safety and security as defined in the Practice Health and Safety policy. Ensure that any hazards in your working area are reported immediately to your line manager.

Performance/Professional Development:

• Training sessions to ensure administrative, clinical and computer skills are kept up to date with current technology and practice.
• Comply with the General Pharmaceutical Council Re-validation process.

Working with Patients:

• Work in partnership with our patients to help them achieve better health outcomes.
• Offer a polite, friendly, and efficient level of customer service to all our patients.
• Provide specialist expertise in the use of medicines while helping to address both the public health and social care needs of patients in the network and help in tackling inequalities.

Feedback:

• Listen to feedback from patients and colleagues whether this is a complaint, comment, or compliment and if it cannot be dealt with immediately, ensure it is reported to a member of the Practice Management Team.

Person Specification

Qualifications

• An undergraduate Masters Degree or equivalent in Pharmacy.
• Membership as practising pharmacist with The General Pharmaceutical Council.
• Commitment towards continuing professional development in accordance with requirements of The General Pharmaceutical Council.
• Pharmacist independent prescriber registered with The General Pharmaceutical Council.
• Post-graduate qualification to Diploma or Masters level in a recognised area of pharmacy practice or advanced clinical practice.

Experience

• Experience working in a primary care setting.
• Knowledge of medicines schemes such as MOF and Eclipse.
• Experience using EMIS clinical system.

Knowledge and Skills

• Effective communication and interpersonal skills.
• Proficiency using computer systems such as Accurx and Microsoft.
