This part-time role offers flexibility, the opportunity to work partly from home, and the chance to work as part of a supportive and collaborative multidisciplinary team.

This is a fantastic opportunity for a qualified pharmacist looking to expand their role in general practice, contribute to high-quality patient care, and be involved in medicine optimisation and structured medication reviews.

Key Responsibilities

  1. Conduct structured medication reviews for patients with chronic diseases and complex polypharmacy, including care home residents and vulnerable patient groups.
  2. Support medicine optimisation initiatives, including the reduction of unnecessary or low-priority prescribing.
  3. Apply specialist knowledge in medicines management to deliver high standards of patient care and contribute to service improvement.
  4. Promote safe, evidence-based prescribing, antimicrobial stewardship, and patient education.
  5. Support the practice team with QOF targets, enhanced services, and patient safety initiatives.
  6. Collaborate with wider healthcare teams (including community and secondary care) to streamline patient pathways and improve health outcomes.
  7. Participate in clinical governance, including audits, significant event reviews, and continuous quality improvement.

Requirements

Essential:

  1. MPharm degree (or equivalent) and registration with the General Pharmaceutical Council.
  2. Independent Prescriber qualification.
  3. Minimum of 2 years' post-registration experience.
  4. Experience managing patients with chronic conditions in a clinical setting.
  5. Strong communication and IT skills (including use of clinical software and Microsoft Office).
  6. Ability to work autonomously and as part of a multidisciplinary team.

Desirable:
  1. Previous experience in a primary care, community, or hospital pharmacy setting.
  2. Postgraduate qualifications in clinical pharmacy or a relevant specialist area.
  3. Familiarity with SystmOne or other general practice clinical systems.

What's on Offer

  1. Flexible, part-time hours (25.5 per week)
  2. Competitive salary (pro rata) based on experience
  3. Remote working available for part of the week
  4. A supportive working environment with opportunities to contribute to innovation and service development
