CoCO Primary Care Network

Position: Primary Care Clinical Pharmacist

COCO PCN is an innovative, dynamic PCN combining 3 local GP Practices covering Addlestone, Chertsey and Ottershaw. We are continually seeking new ways to improve local delivery of healthcare services for our population to enable residents to live the healthiest lives possible.

An exciting opportunity has arisen for a Clinical Pharmacist to join our multi-disciplinary team within CoCO. It is a requirement of the role to be registered with the General Pharmaceutical Council along with Membership of the Royal Pharmaceutical Society. Primary care experience would be an advantage but is not essential. Flexibility, enthusiasm, and the ability to prioritise tasks effectively are essential skills.

Hours: 37.5 Hours per Week (will consider job share or part time)

Rate of Pay: Dependent on experience

Responsible to: CoCO Primary Care Network

Main Purpose of the Post:

To be responsible for, leading on, and supporting the implementation of an effective medicine management system within the PCN, including cost-effective prescribing, working as an independent prescriber, and providing advice and support to multi-disciplinary colleagues. In addition, the post-holder will also work with the strategic objectives of CoCO PCN specifically in relation to supporting medical review work in our care homes.

The role will also include training and development of practice-based prescribing clerks and administrators to support improved coordination and effective pharmacy administration within general practice, especially concerning repeat prescribing systems and processes. You will also be expected to:

  1. Develop and facilitate a good working relationship with community pharmacists and other local providers of healthcare.
  2. Plan and organise your own workload, including audit and project work, and training sessions for members of the practice team, community pharmacy team, community nurse team, patients, carers, etc.
  3. Record and maintain a database of information relating to the work done in the PCN, including any work relating to audits undertaken by the post-holder and reference notes relating to clinical/technical information.
  4. Maintain registration as a pharmacist and comply with appropriate professional codes.
  5. Maintain and develop professional competence and expertise, keep up to date with medical/therapeutic evidence and opinion, and local and national service, legislation, and policy developments, agree objectives and a personal development plan, and participate in the appraisal process.
  6. Attend local, regional, and national meetings of relevance.
  7. Undertake any other duties commensurate with the post-holder's grade as agreed with the post-holder's line manager.
  8. Understand that it is the personal responsibility of all employees to comply with all organisational and statutory requirements (e.g. health and safety, equal treatment and diversity, confidentiality, and clinical governance).

Job Types: Full-time, Part-time, Permanent

Pay: £35,000.00-£45,000.00 per year
