Job Summary
The Clinical Lead will support the Operational Team Lead in leading a multidisciplinary team in the delivery of a clinically effective Nursing Service, providing assessment, treatment, and Care Co-ordination to patients in the local community referred to the Community Mental Health Team. Responsible for the pro-active case management of a wide range of patients, many of whom have complex needs. Accountable to the Health Board Nurse Director and the Nursing and Midwifery Council (NMC) to provide safe, high-quality care by implementing policies/standards and guidance from NMC, Welsh Government, Powys Teaching Health Board, and others. The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

Main Duties of the Job
The person appointed will be expected to undertake a range of duties to include:

  1. Being responsible and accountable for an identified caseload, prescribing, providing supervision to the team, liaising with partner agencies, dealing with concerns, chairing and engaging in MDT meetings.
  2. Monitor standards of practice within the team, recruitment and appointment of staff, absence management, and the deployment and allocation of duties.
  3. Work with the team to ensure high standards of patient care and maintain safety and quality of the patient experience.
  4. Lead the implementation and monitor local and national nursing policy, procedures, and guidelines to support best practice and ensure adherence to current legislation.
  5. Use expert knowledge to manage and deliver care for patients with highly complex and unstable health needs within the community setting.
  6. Ensure delivery is safe, efficient, and effective.
  7. Responsible for accurate recording, timely collection, completion, and submission of relevant information to support patient care, operational management, nursing staff, skill mix, service development, and audit.
  8. Undertake a training needs analysis for nursing within your team and implement agreed training plans.
  9. Actively participate in the research and development agenda.

Person Specification

Qualifications and/or Knowledge

Essential

  • Live registration with the NMC as Registered Nurse
  • Degree/Diploma in Nursing
  • Post registration qualification/significant experience in Mental Health
  • Management/Leadership qualification or equivalent preparation/experience
  • Knowledge and skills to manage patients with complex needs
  • Knowledge of current Nursing issues within NHS care systems
  • High level of knowledge of NMC code of conduct and professional standards/guidance

Desirable

  • Independent Prescribing Qualification
  • MSc Advanced Clinical Practice

Experience

Essential

  • Experience at management level demonstrating ability to deliver improvements to service user care
  • Experience of proactive caseload management of a wide range of patients

  • Ability to use computer systems (email, word processing, spreadsheets)
  • Significant clinical experience in assessing, planning, implementing, and evaluating care for the patient group of the caseload
  • Well-developed verbal and written communication skills
