Job summary

An exciting opportunity has arisen for a Clinical Exercise Physiologist to join our friendly, passionate, and nationally accredited multi-disciplinary Cardiac Rehabilitation Service.

We are looking for a motivated individual with excellent communication and interpersonal skills to play a key role in delivering high-quality, patient-centered rehabilitation services.

As a Clinical Exercise Physiologist, you will:

1. Assist in delivering multi-morbidity clinical rehabilitation services across Coventry and Rugby.
2. Hold a clinical caseload within the cardiac rehabilitation team.
3. Work across multiple sites, including the Centre for Exercise and Health (Coventry) and St Cross Hospital (Rugby).
4. Support the development and implementation of new and innovative rehabilitation services in clinical practice.

2. Experience in working with cardiac or multi-morbidity patients.
3. Ability to work within a multi-disciplinary team.
4. Passion for improving patient outcomes through exercise-based interventions.

Main duties of the job

As part of our commitment to patients and delivery of a world class service for all we have created the UHCW Improvement (UHCWi) System in partnership with the Virginia Mason Institute in Seattle; this involves a structured approach to removing waste and putting the patient first using a lean management system and methodologies. You are expected, where identified, to attend and complete relevant training and development opportunities to support this. This may include Lean for Leaders, Advanced Lean Training, and the Human Factors Programme, amongst others. Full attendance and completion of identified courses is considered essential and a pre requisite for this post.

1. A skilled and knowledgeable practitioner in exercise science applied to a range of long-term conditions and morbidities including cardiovascular, metabolic, respiratory and cancer.
2. Accountable for own actions in accordance with the REPS and NHS Code of conduct.
3. Assist in the management and day to day running of the community and hospital multi-morbidity rehabilitation programme.
4. Participate in the development of quality initiatives, including standards of the multi-morbidity rehabilitation programme, patient care, clinical audit, monitoring of service provision and clinical research.

Job responsibilities

1. Ability to undertake a clinical assessment (as an autonomous specialist) of patients with long-term conditions, including those with complex presentation, investigate, analyse & formulate individualised management & treatment plans to formulate specialised programmes of care.
2. To identify and respond appropriately to additional long-term conditions and/or co-morbidities and the potential effect these may have on physical activity and lifestyle advice. Use this information appropriately prior to advising, planning or implementing physical activity and lifestyle treatment plans.
3. Accept clinical responsibility for own caseload & organise this effectively & efficiently with regard to the clinical progress of the patient.
4. Maintain immediate life support and defibrillation training ILS.
5. Collaborate with long term condition specialists and all members of the multidisciplinary team in the primary and secondary health care settings to ensure patients are receiving the highest quality of care.
6. Ensure patient information is collated, monitored and fully accurate maintaining complete records in line with Trust standards.
7. Promote the involvement and engagement of patients, members and carers/relatives in the assessment, delivery and evaluation of care.
8. Maintain and develop clinical expertise by attendance and participation in regular post-registration education as identified by PDPs i.e. BACPR/BASES study days or conferences and in-service training, participating in mentorship and appraisal as required.
