The Senior Palliative Care Nurse role will support outreach into the community (working with the CCN teams etc.), in-reach into the acute hospitals (having a regular presence on wards, attending ward rounds etc.) and development of community hubs, building relationships with professionals, and raising awareness of all the services/support we can provide. This is an important step forward in the development of our services and this role is pivotal to this work. Therefore, we are looking for talented individuals who share our passion to improve the lives of children and young people with a variety of complex health conditions.
