Children's Community Nursing Clinical Lead

You will be key in clinically leading a multidisciplinary Children's and Young People's Community Nursing Team that engages across multiple health care partners. You will contribute to the clinical development of the Team, whilst prioritising specialist practice effectively to support the demands of the Service, focusing on the health and wellbeing of children on the caseload. You will have had previous clinical expertise within Children Services, alongside proven and compassionate leadership skills, and be able to promote and work within a culture of innovation and quality improvement that supports Children, Young People and their families to remain at home.

We are delighted to offer this newly created post to support the clinical delivery of the Children's and Young Person Community Nursing Team in Brighton and Hove. As a Registered Nurse- child, you will work alongside clinical and operational colleagues to provide clinical guidance and support the team which focuses on the public health nursing of children and young people and families across Brighton and Hove.

This role will sit within the Leadership team for the Brighton and Hove Children's and Young People's Community Nursing Team, which is one of the teams that comprises the Sussex Wide Children's and Young Peoples Community Nursing Service. The role supports and deputises for the Band 8a Matron role, working alongside the Service's Team Lead and Practice Development Nurses. The role is to ensure the smooth clinical delivery of the service, working across professional and clinical disciplines, aligning clinical standards with operational performance.

As the Clinical Lead in Children's Community Nursing is a clinical leadership role within Sussex Community Trust (SCFT) Children and Young Peoples Community Nursing Service (CYPCNS) you will have both advanced and specialist expertise in clinical, children's nursing care, research, audit, practice development and educational support. You will contribute to the development of the countywide Children and Young Peoples Community Nursing Service plan and together with other senior nurses, will ensure policy and practice is developed and delivered in line with Trust and national best practice standards, and will undertake priority clinical projects to improve service provision both locally and Trust wide.

As a Clinical Lead you will promote the health and wellbeing of children/young people and their families to ensure the provision of high quality, safe, clinical nursing care to children and families on the CCN caseload in collaboration with the Team Lead. You will hold your own independent clinical caseload and be proficient in clinical nursing assessments and interventions for children/young people in the community. This is a dynamic role able to flex to meet the needs of children and families on the caseload. You will work closely with other health professionals within the Trust and across the health economy, as well as across statutory and third sector agencies.

This role offers a holistic, streamlined service that is easy for patients and families to access and can prevent or shorten hospital admission. You will protect, safeguard and promote the well-being of children in accordance with The Children Act (1989 & 2004) and Working Together to Safeguard Children (HM Gov. 2013). You will provide education, training and competency assessment of clinical nursing skills for colleagues, students, parents, carers as well as children and young people on the caseload.

The Clinical Lead within Children's Community Nursing is, or will undertake training to become, a Non-Medical/Independent Nurse Prescriber (V300) who can offer clinical reviews, assessments and prescribe appropriately for children in the community. You will then prescribe within the limits of their professional competency and Trust guidelines.

As the Clinical Lead within Children's Community Nursing you will:

• Provide, receive and understand complex, or sensitive information relating to children and young people with a range of health needs. This will include children with highly complex long term conditions and those children with life threatening and life limiting conditions.
• Communicate information effectively with members of the multi-professional team both within the organisation and with external stakeholders and partners, to ensure specialist care is delivered to children both effectively and efficiently and ensuring that continuity is maintained.
• Work with children and families to help maintain their own health and well-being in the community by teaching them and junior staff/carers about their condition and its management.
• Communicate complex and sensitive information effectively to children/young people and their families and other staff overcoming any barriers to understanding.
• Lead and support team in co-ordinating the discharge process and ensures high standards of written and verbal advice are available to the children and their families / carers.
• Ensure that the Team Lead, Matron and/or General Manager are informed of pertinent clinical and management problems in a timely manner.
• Report and escalate as necessary any safeguarding or patient safety concerns for the children on the caseload according to trust and local policy.
• Ensure systems are in place to proactively disseminate information arranges and facilitates staff meetings.
• Work in partnership with PALS and senior management to provide resources within the CCNT, for the management and local resolution of patient related complaints.
• Act as an ambassador / role model for Sussex Community NHS Foundation Trust and the Nursing Profession.

• Support the Team Lead with the recruitment of staff into the team.
• Support the Team lead with budgetary responsibility for the team.
• Support the team lead with ensuring all Health and Safety requirements are met.
• Lead the Team to take part in the End of Life Care on-call rota.
• Set and maintain clinical and service performance standards.
• Lead and manage the day to day operational nursing, personnel, performance and allocation of work.
• Work in partnership with the team lead on staff development and performance within the team.
• Educate and support staff offering direction and leadership surrounding continual professional development.
• Lead on change management within the team.
• Ensure health records are maintained and compliant with SCFT Trust policy.
• Assist the Named Nurse for Child Protection in co-ordinating child protection processes.
• Initiate, maintain and analyse relevant service data.
• Regularly evaluate, with the team lead, the clinical effectiveness of care.
• Develop, implement, and evaluate guidelines/teaching tools for children/families/colleagues.
• Participate in nursing research and audit projects.

The successful candidate will join a Team which delivers excellent Children's nursing services to the residents of Sussex, working in collaboration with our system partners across health, social and voluntary care. You will have a full valid driving licence and access to a car for work purposes.

• Registered children's nurse
• BSc/MSc Specialist Practice in Community Health/Specialist Practitioner Children's Community Nurse or BSc/MSc Children's Nurse Practitioner
• Physical Assessment of Children/Young People at level 6/7 or equivalent
• Relevant study at Level 7 & willing to work towards relevant Masters qualification
• ENB 998 / Mentorship module or equivalent
• Community Nurse prescriber (V100) or willing to undertake training
• Non-Medical/Independent Nurse Prescriber (V300) or willing to undertake training
• Specialist Practice Teacher/ PGCE or willing to undertake
