I am currently with Achieving for Children, who are offering an initial temporary contract for a period of 3-6 months with a view to be extended or offered a permanent position for the right candidate at a rate which is negotiable dependent upon experience.
We are looking for a Children's Community Nurse (Band 6) for the role located in KT9 2AA.
The right candidate will:
- Be an enthusiastic, proactive nurse to work within our Community Nursing Team.
- Work autonomously across the borough to provide holistic, child and family centered care for children and young people with SEND, PMLD and complex health needs, ranging from 2 - 19 years of age.
- Engage in a dynamic role encompassing: assessment, planning, delivery, and evaluation of clinical nursing care; writing appropriate care plans to ensure safe and effective care delivery.
- Deliver specialist nursing care, including but not limited to gastrostomy management, wound care, and suction.
- Provide specialist training for families and school support staff, and engage in multi-disciplinary liaison to ensure continuity of care, including attendance at multi-professional meetings, such as child protection case conferences.
- Support families by signposting to specialist services and completing onward referrals for care, while providing health advice for education, health and care plans.
