Children&Rsquo;S Community Nurse (Band 6)

Good Morning,

I am currently with Achieving for Children, who are offering an initial temporary contract for a period of 3-6 months with a view to be extended or offered a permanent position for the right candidate at a rate which is negotiable dependent upon experience.

We are looking for a Children's Community Nurse (Band 6) for the role located in KT9 2AA.

The right candidate will:

1. Be an enthusiastic, proactive nurse to work within our Community Nursing Team.
2. Work autonomously across the borough to provide holistic, child and family centered care for children and young people with SEND, PMLD and complex health needs, ranging from 2 - 19 years of age.
3. Engage in a dynamic role encompassing: assessment, planning, delivery, and evaluation of clinical nursing care; writing appropriate care plans to ensure safe and effective care delivery.
4. Deliver specialist nursing care, including but not limited to gastrostomy management, wound care, and suction.
5. Provide specialist training for families and school support staff, and engage in multi-disciplinary liaison to ensure continuity of care, including attendance at multi-professional meetings, such as child protection case conferences.
6. Support families by signposting to specialist services and completing onward referrals for care, while providing health advice for education, health and care plans.
