A unique position has become available as we expand our Home Care Package portfolio within Northern Tasmania. We're looking for a fulltime Case Manager to join our dynamic team.

The Position:
The Case Manager is a full-time position based in Northern Tasmania with flexibility. The successful candidate will be responsible for the initial provision and case management of Home Care Packages for clients. The position is pivotal to directing client care covering appropriate intake and assessment, care planning, community engagement and documentation, monitoring and review, fund management and advocacy as required.
A dynamic role within our Tasmania team, working in close partnership with the Home Care Clinical Lead. You will form part of a collaborative, agile team with a refreshing approach and culture, as we strive to support our clients to ‘live life as they choose’.
To be considered for employment with Respect, each candidate must satisfy the legislative requirements of Aged Care Worker Screening to ensure suitability to work in the aged care industry.
