Sanctuary Personnel, an innovative and committed recruitment agency, has a new permanent position available for an Adult Care Coordinator within the Integrated Learning Disability Team to work full time based in Enfield.

Based in the bubbling heart of Enfield Town, Enfield’s recently refurbished open plan office has bespoke areas for service users and is an environment which supports their smart working policy surrounded by small businesses, bars, restaurants, a buzzing high street, and shopping centre. The high-tech multi-agency office has good transport links into Central London and is easily accessible via the M25, A10, and A406.

The salary for this permanent Care Coordinator job is up to £32,805 per annum.

This is an exciting opportunity to be based within a local authority integrated health and social care service. The service comprises many disciplines including Social Work, Community Nurses, Psychologists, Occupational Therapists, Speech and Language Therapists, Art Therapists, Physiotherapists, and Psychiatrists. The Care Coordinator post will sit within the social work team.

The role entails face-to-face work with service users, their families, and external partners. The purpose of the Care Coordinator role is to identify whether support packages continue to meet the needs of service users and their families. The role will require the successful candidates to make recommendations regarding support packages and arrange and amend support to ensure needs are being met in a person-centred and strength-based way.

The role also requires the successful candidates to be part of our integrated front door duty service on a rota basis, supporting with resolving crisis situations.

An exciting part of the role will include undertaking assessment work and supporting with safeguarding case work.

You will need excellent communication skills, both verbal and written. The Care Coordinator will be required to attend multi-disciplinary meetings and present cases, communicate effectively with service users and their families, as well as a range of internal and external colleagues and partners. In addition, the Care Coordinator will be required to write accurate, comprehensive support plans and assessments.

Main duties:

  • To provide assessment, care management, and social work services to users and carers arising out of the council’s duties and powers under legislation, Council, and Group Policy. To work in accordance with the guidelines set out by local management and to make appropriate use of Group and Independent resources.
  • To maintain high professional standards at all times.
  • To undertake initial assessments within the area of specialism.
  • To manage, monitor, and review care plans involving users and carers and liaising with other workers and agencies as appropriate.
  • To work at all times within the council’s Financial Regulations.
  • To be aware of the resources available within the specialist area, including other sources of funding so that the maximum benefit may be obtained for service users within budgetary resources of the group.
  • To make use of the management information systems, including information technology, in the recording retrieval and analysis of information as required by the department.
  • To be managed and receive supervision from the immediate line manager and to be assessed against agreed standards and targets.
  • Carry out accountabilities in compliance with the councils policies and procedures.
  • Any other duties reasonably requested by management.

Requirements of this Care Coordinator job:

  • As a regular and intrinsic part of this role requires you to speak to members of the public in English, the ability to converse at ease with customers and provide advice in accurate spoken English is essential and consistent with the requirements of this role. This role also requires you to be polite and courteous when conversing with the public.
  • Ability to assess need and to make appropriate support plans and have a good understanding of the care management process.
  • Ability to work in partnership with service users, carers, and other professionals whilst maintaining the service user at the centre of each process or activity.
  • Ability to work with safeguarding cases and understanding the safeguarding processes.
  • Ability to work within the departmental financial policies and controls, developing best value services and community placements and working with service users, carers, and those professionals connected to each individual and or service provider.
  • Ability to show the following key behaviours: 1. Taking responsibility for one’s actions; 2. Being open, honest, and respectful; 3. Listening to others and being open to learn; and 4. Working together with others to find solutions.
