Job description

We are recruiting for a new Care Coordinator to join our Sussex branch within Unity Care Solutions.

The Care Coordinator is an operationally critical role within the company, functioning to ensure the effective and efficient planning and delivery of care. The Care Coordinator will be proficient in prioritising and multitasking, all the while making sure the client’s safety and quality of care is not compromised at any time.

This is a client and staff-facing role where you will represent the values of the company each and every day, therefore a friendly and professional disposition is essential. You will be a central support to our care and nursing staff, and will liaise with our clients and their families to make sure that all care is arranged in accordance with their needs.

Duties will include:

  1. Act as the primary point of contact for clients and their families, providing support and guidance throughout their care journey.
  2. Coordinate the planning and delivery of care, including creating rosters; assigning shifts; monitoring of availability; and coverage of staff absence.
  3. Continuous effective, meaningful and professional communication with care staff about care packages, their requirements, and any other relevant information.
  4. Prepare and produce weekly payroll through collation and verification of timesheets and expense claims.
  5. Support care staff, including through performance and absence monitoring processes.
  6. Involvement in the recruitment and induction processes for care staff.
  7. Visit clients and their families to conduct quality monitoring visits.
  8. Distribution and collation of paperwork.
  9. Monitoring and updating of digital systems.
  10. Support and monitoring of employee and care compliance requirements.
  11. Attend and participate in regular team meetings, sharing key updates on client activity and assigned actions.
  12. Provide general administrative support, including answering telephones, filing, photocopying, minute-taking, and anything else that would reasonably fall under this definition.

This role also requires participation in the out of hours on-call roster for emergency operational support.

In return, we provide:

  • An achievable performance-related bonus scheme.
  • Complementary membership to Benenden Healthcare.
  • On Call pay enhancements for weekends and bank holidays.
  • Access to our wellness programme, including 24/7 access to an online GP service.
  • Blue Light card discounts.
  • Ongoing training opportunities.

About Unity Care Solutions

Unity Care Solutions deliver care and clinical nursing to adults and children who have complex long-term health conditions within the comfort and safety of their own homes.

We are regulated by the Care Quality Commission, which supports our high standards for excellence in everything we do. We are experiencing a period of growth, and are developing our business support functions in response.

Not only will you have the opportunity to work with a group of colleagues who will support you in your role, but you will become part of a company that truly cares about its people. We place a huge emphasis on the wellbeing of our employees through a company-wide wellness programme and actively promote participation in events and initiatives.

About You

You will ideally have experience of working within a similar role within health & social care; however, this is not essential.

What is essential is that you can demonstrate experience of working in a role which requires acute coordination, prioritisation, and organisation. You will be used to working within a fast-paced environment and will have a solution-focused attitude when the unexpected happens.

You will be as keen to help as you are keen to learn and will have a positive can-do attitude when working within the team.

As you will be visiting our staff and clients, you will need to have a Full UK Driving Licence, access to your own vehicle, and the relevant motor insurances.

If you would like to apply or learn more, you can email jobs@unitycaresolutions.co.uk, or call us for an informal chat on 0333 366 1020.
