We are seeking a dedicated and experienced Care Coordinator to join our growing team. This role is essential to ensuring that our clients receive the highest quality of care and that our support workers are well-supported and motivated.
As the Care Coordinator, you will act as the primary point of contact for clients and support workers, providing critical operational support and overseeing daily care logistics. Your key responsibilities will include developing and managing care schedules, monitoring and addressing cancelled care hours, overseeing high-quality, person-centred care, and maintaining strong communication with clients and families. You will also be responsible for recruiting and matching support workers to clients, collaborating with HR and Clinical Leads to address staff issues, and ensuring ongoing training. Additionally, you will coordinate with internal and external stakeholders, supervise care and medication standards, maintain accurate records, and report on client progress and service impact.
