Job description

Care Coordinator - Home Care (Private Clients)
Location: Office-Based, Ware SG
Salary: £29,500 per annum + On-Call Allowance
Hours: Monday - Friday, 8:00 am - 4:00 pm OR 9:00 am - 5:00 pm (Office-Based)
On-Call: Rota Basis - Maximum 7 days per month (Additional Pay)

Are you an experienced and compassionate care professional ready to take the next step in your career? Join our client in their busy, supportive, and dedicated branch delivering over 1,200 hours of private home care each week to their valued clients.

About the Role:

As a Care Coordinator, you'll play a vital role in ensuring clients receive exceptional care that's well-planned, consistent, and tailored to their individual needs. You'll be the bridge between clients, families, carers, and internal teams - keeping everything running smoothly, professionally, and with heart.

Your responsibilities will include:
  1. Coordinating care hours and schedules
  2. Liaising with clients, families, carers, and colleagues
  3. Writing and updating person-centred care plans
  4. Handling on-call duties on a rota basis (maximum 7 days per month with additional pay)
  5. Supporting the branch to maintain high standards of care delivery

What We're Looking For:
  1. NVQ Level 2 or 3 in Health & Social Care (Essential)
  2. A full UK Driving Licence (Essential)
  3. Previous experience in care coordination or senior care roles
  4. Strong organisational skills and attention to detail
  5. Excellent communication and relationship-building abilities
  6. A proactive and supportive approach to teamwork
