Key Care & Support are a specialist provider of person centred community support, supported living, respite and care at home support to adults and children with complex and individual needs.

Job Opportunity

We have an exciting opportunity for a Part time Care Coordinator to join our highly successful team in Manchester.

Overview

The role of Care Coordinator will be to provide a positive and effective focused service to a designated group of people we support as directed by the Line Manager.

Hours will be approx 20-24 hours per week, with some flexibility to meet the business needs. Days and hours will be discussed at interview.

You would need to be a driver and have your own transport for this role.

Main Duties
  1. To provide a positive focused experience to a group of clients.
  2. To assess, plan and review support plans of people we agree to support.
  3. To positively and proactively liaise with support staff to ensure continuity of support and care is maximised.
  4. To liaise with commissioners and associated professionals to procure community support packages and complex care packages.
  5. To provide a proactive service for all customers, ensuring effective communication is central to role and function.
  6. To alert the Line Manager to any concerns raised.
  7. To liaise with the Line Manager on what roles are required to fulfil support package requirements.
  8. To inform the Line Manager of any complaints or compliments received and follow instruction on how to proceed.
  9. To ensure staff are respected as our internal customers and are valued as an essential aspect of the business.
  10. To ensure Webroster, Log my Care and related documentation is kept up to date at all times.
  11. To ensure Care and support packages are 100% filled at all times.
  12. To ensure shifts are filled to capacity.
  13. In emergency situations, deliver support and care to ensure the people we support are safe and protected.
  14. To provide overnight emergency on-call support on a rota basis as required.

This Job profile sets out the main duties required to fulfil the role but is by no means exhaustive. Additional requirements in line with the role will be required of the postholder as requested by your Line Manager.
