Job Overview
We are seeking a dedicated and compassionate Care Coordinator to join our team. The Care Coordinator will play a vital role in ensuring that clients receive the highest level of care and support throughout their healthcare journey. This position involves working closely with care team, clients, families, and healthcare providers to facilitate communication. The ideal candidate will possess strong organisational skills and a commitment to improving client outcomes.

MAIN RESPONSIBILITIES

  1. Support to ensure carers rotas are released weekly.
  2. Support to ensure clients receive their schedule of care visits and carers each week.
  3. Support Head of Coordination to ensure all contractual service orders and times are adhered to; if changes are required in the event of an emergency, ensure correct reporting procedures are followed.
  4. Support the compliance team to enable client/carer introductions by scheduling visits.
  5. Responsible for ensuring ‘Live view’ is monitored and concerns actioned and recorded immediately when on duty.
  6. Responsible for the safety of our care teams while monitoring ‘Live view’ when on duty.
  7. Responsible for the safe management of medications through monitoring ‘Live view’ when on duty.
  8. Responsible for ensuring sufficient travel time is given between visits.
  9. Monitor, record, and report regular concerns regarding ‘Call cutting’.
  10. Support Head of Coordination to ensure our sponsored workforce are working their minimum hours of 39 hours per week and reporting any concerns to the Senior compliance manager.
  11. Responsible to support the Head of Coordination in ensuring carers are receiving required and adequate breaks and time off to ensure quality care can always be delivered.
  12. Responsible to support the Head of Coordination in ensuring any Sponsored Students do not work more than 20 hours per week during term time and gain confirmation from the HR Manager when their student holidays are.
  13. Understand your own responsibilities regarding CQC requirements, Local authority requirements, and the local safeguarding teams.
  14. Responsible for completing ‘On Call Hub’ duties as required.
  15. Ensure all compliments/complaints/accidents/incidents are correctly responded to, reported, and recorded as appropriate to your role, and ensure the correct team has been notified.
  16. Ensure the On-Call log is completed, and a handover meeting is recorded daily, ensuring the appropriate teams are informed using the correct channels.

ON CALL HUB

  1. Responsible for following the guidance as stated in the ‘On Call Hub’ manual at all times.
  2. Answer all phone calls to both clients and carers in a polite and professional manner, and return voice messages within 15 minutes.
  3. Deal with emergencies that may arise while on call, and record and report as appropriate.
  4. Support the coordination team to always ensure adequate cover, cover for annual leave of colleagues as required to ensure consistency.
  5. Complete and maintain the ‘On Call Log’ for every shift and participate in handover meetings as required and directed by the head of coordination.
  6. Complete correct communication logs of events/Zendesk tickets to the appropriate individuals/teams.

Other responsibilities:

  1. Maintain Health & Safety standards in accordance with UK Care Teams policies and procedures, including undertaking risk assessments for all care-related tasks, environments, and medications.
  2. Complete all mandatory and CPD training as required and/or requested by line manager, or any further development your line manager feels appropriate to your role.
  3. Establish and maintain positive relationships with clients, clients’ families, and professionals.
  4. Ensure up-to-date, appropriate, and accurate record keeping is maintained.
  5. Maintain and understand your own responsibilities in respect of infection prevention and control.
  6. Maintain and understand your responsibilities in respect of safeguarding adults and children.
  7. Responsible for ensuring your own and your team's training and CPD is up to date.
  8. Other responsibilities may be requested of you by your line manager as required in line with the needs of the business.

Qualifications

  • Previous experience in a care coordinator role.
  • Excellent organisational skills with the ability to manage multiple tasks effectively.
  • Strong communication skills, both verbal and written, with an emphasis on empathy and active listening.
  • Proficient in using office software applications for documentation and communication purposes.
  • Ideally experience of using Access care planning & People planner.
  • Ability to work collaboratively within a multidisciplinary team while advocating for client needs.
  • A commitment to maintaining confidentiality and adhering to ethical standards in client care.

Job Type: Full-time

Pay: £24,000.00-£26,500.00 per year
