Location: Manchester, NH 03101

Job Type: Full-time, 40 hours per week

Sign-On Bonus: $5000 for all clinical positions.

Job Description:

ITT is a dynamic, community-based team providing short-term, solution-focused case management to individuals with behavioral health issues who face barriers to sustainable healthcare, with a particular focus on the integration of services with local primary care providers. ITT Case Managers are responsible for providing direct care and case management services. The position requires a Bachelor’s degree in a related field with at least one year of experience working with individuals with psychiatric/co-occurring disorders.

Key Responsibilities:

  • Provide a range of interventions to individuals with mental illness who are in the process of transitioning to longer-term support.
  • Utilize a trauma-informed approach and other evidenced-based practices to engage individuals referred to ITT, build rapport, and assist with solution-focused, goal-directed activities.
  • Administer core standardized assessments to individuals referred for ITT services.
  • Work collaboratively with ITT clients and other members of the multi-disciplinary core team to develop a solution-focused shared care plan.
  • Coordinate and monitor activities outlined in the shared care plan, maintaining a person-centered approach.
  • Provide psychoeducation and skills training focused on empowering ITT clients.
  • Maintain a caseload of ITT clients, practicing effective time management and organizational strategies.
  • Follow all policies and procedures related to referrals, assessments, clinical practices, documentation, and data reporting.
  • Attend core team meetings/huddles and other department meetings, as necessary.
  • Connect and consult with community agencies and natural supports to ensure proper coordination of the service process.
  • Provide services that are relevant to the culture of the consumer and family, including identifying and addressing disparities in healthcare access and quality.

Education:

A bachelor’s degree in psychology, counseling, or a related field is required.

Skills and Competencies:

  • Knowledge of/experience with integrated healthcare practices and community systems.
  • Must have organizational, written, and verbal skills.
  • Basic fundamental computer experience to process e-mail and client access is required.
  • Bilingual candidates are encouraged to apply.

Qualifications:

  • Demonstrated knowledge of social and human service providers, public benefits systems, and referral processes.
  • Experience providing case management and individual assessments to clients.
  • Documented completion of training in any of these areas is a plus: Critical Time Intervention (CTI); Stages of Change Training; Motivational Interviewing; Ethics; Cultural Competence; Person-Centered Planning; Suicidal Prevention.
  • Current unrestricted driver's license and current personal vehicle insurance.
  • A bachelor’s degree in psychology, counseling, or a related field is required.
