Shropshire Community Health Trust is offering an exciting opportunity for motivated individuals with a passion for the specialist field of complex discharge to work within the Integrated Health and Social Care Discharge Team at the Shrewsbury and Telford Hospital Trust as a Bank 6 Senior Discharge Coordinator. The positions are an ideal opportunity for Nurses, Social Workers, and Allied Health Professionals to shape this new service for our patients. The team is expanding and this is an ideal opportunity to be involved in the transformation of the service and work with the existing skilled team in the specialist field of Complex Discharge.

The role and service offer integration and partnership working with team managers, nurses, therapists, and health and social care coordinators across both organisations to ensure a seamless service.

Main duties of the job

The Senior Discharge Coordinator holds specialist knowledge in this field and will work within the Integrated Discharge Team providing an expert and skilled community-focused approach to discharge planning and assessment, embracing the principles of the Discharge to Assess Model and providing a person-centred approach to care. As the Senior Discharge Coordinator, they will be responsible for leading the team on a daily basis, prioritising daily workload, case managing the most complex discharge cases, coordinating daily capacity, and supporting patient flow while working with all key partners, using their expert knowledge of community services to create a seamless pathway of care from hospital to community and home.

The successful applicant will be supported by the Community and Acute Hospital Service Managers in the Integrated Discharge team's service development, working with the team in clinical, health care, and social care practice, providing knowledge, skills, and experience of acute and community health care to our patients to achieve timely and safe discharge from hospital.

The service operates 8 am - 6 pm, 7 days a week; this post includes bank holidays and weekend working as part of the clinical team rota.

  • Ability to meet the travel needs of the post.

Training and Qualifications

  • Professional Registration (NMC, HCPC, or DipSW).
  • Professional knowledge acquired through degree-level qualification or significant equivalent experience.
  • Diploma-level specialist training, experience, or short courses in the appropriate field.

Experience

  • Demonstrable experience of contact with patients both in an acute and community setting.
  • Demonstrate knowledge of community and social care services to support hospital discharge pathways.
  • Experience of working with multidisciplinary health and social care teams.
  • Experience of developing systems, processes, and new ways of working to optimise patient flow.
  • Understanding of current mental health legislation and safeguarding processes.

Knowledge and Skills

  • Knowledge of the Discharge Pathways.
  • IT skills in the use of Microsoft products (Word, Excel, Outlook, Publisher, PowerPoint, etc).
  • Articulate with well-developed communication (verbal and written) and interpersonal skills.
  • Able to prioritise and meet deadlines effectively.
