Job summary

BHRUT are committed to changing the model of care for our increasing elderly and frail population by shifting to a more proactive care approach. By doing so we want to identify those people at risk of developing frailty and initiate early interventions to improve management, prevent deterioration and even reverse a person's level of frailty.

At the heart of this will be a multi-professional comprehensive geriatric assessment. We will be implementing further measures to proactively manage patients who may be heading towards a crisis that would otherwise result in attendance to hospital, by providing same and next day urgent assessment appointments. Where patients require short term intravenous therapy, we will provide the facility to rapidly respond to this need.

We have big ambitions to radically change the provision of care for the ageing population of Barking, Havering, and Redbridge and we are looking to recruit a skilled workforce to help us deliver the vision. Main duties of the job

The post holder will be responsible for providing and developing comprehensive, patient orientated services for frailty across Barking Havering and Redbridge University Trust. They will work closely with the lead Pharmacist for Frailty and out of hospital care to further innovate and develop the frailty service with the aim of reducing attendances to the emergency department, reduce acute admissions into hospital and ensure the safe and effective use of medicines for our elderly population.

Our patients are benefitting from our Women's Health Hub in Ilford; an Ageing Well Centre in Hornchurch; and Community Diagnostic Centres (CDC) at Barking Community Hospital and at St George's Health and Wellbeing Hub in Hornchurch.

These CDCs are open 12 hours a day, 7 days a week and are providing an extra 88,000 scans a year.

The majority of our 8,000 staff - who come from 146 different countries - live in the three diverse London boroughs we serve and are from black, Asian and minority ethnic groups. Many can work flexibly and more than 400 of them are on our Ofsted accredited apprenticeship programmes. We're proud to be a London Living Wage employer.

  • Comprehensive experience as a clinical pharmacist practitioner - this must include a solid grounding in all aspects of hospital pharmacy practice.

  • Change management and quality improvement experience and direct involvement of redesign within a service, including promoting evidence-based practice.
