What you will do:

• Provide a comprehensive clinical pharmacy service to inpatient wards at the City Hospital Campus.
• Conduct medicines reconciliation to ensure patient safety and accuracy in medication records.
• Offer patient counselling to enhance understanding and adherence to medication regimes.
• Prepare TTOs (To Take Out) ensuring patients have the correct medication upon discharge.
• Deliver specialised medicines advice to the wider multidisciplinary team.
• Collaborate with Medicines Management Technicians to provide optimal care in medication management.
