These new roles will be part of our developing Transfer of Care Hub where you will work with colleagues from local authorities and the voluntary and community and social enterprise sector.
The Hub aims to ensure we provide people with the right care within the community so that they aren't in hospital any longer than is necessary.
Please note: This post is open to newly qualified social workers. In line with AWP trust policy all newly qualified social workers (NQSW) will be recruited initially at Band 5 level, regardless of advertised role banding. All NQSW's are expected to successfully complete their ASYE within 2 years of employment, unless there are exceptional circumstances in which case it may be extended for a further 2 years.
If you have been appointed to an advertised Band 6 role this will be an automatic progression, following confirmation of your successful ASYE completion. If you have been appointed to a Band 5 post you will then be eligible to apply for a subsequent Band 6 vacancy upon successful ASYE completion.
As a Clinical Case Reviewer, you will play a crucial role in ensuring the quality and effectiveness of our mental health services. Your responsibilities will include:

1. Clinical Case Management
1.1. Identify and risk-stratify vulnerable high risk service users requiring new specialist placements, by working with clinical teams and commissioners.
1.2. Attend Care Programme Approach (CPA) / the revised Your Team, Your Conversation, Your Plan framework and Multi-Disciplinary Team (MDT) ensuring a clear and appropriate discharge pathway can be worked towards, in a timely way, so as to maximise opportunities for gaining independence and moving back into the community.
1.3. Ensure commissioned providers are delivering against excellent quality standards of patient care and that outcomes for individuals placed in specialist settings are being achieved. Establish robust ongoing quality and outcome monitoring processes; taking appropriate action to improve any underperformance.
1.4. Monitor placement / care costings to ensure ongoing value for money, and that care and treatment delivered is reflective of service user need. Where this is not the case, support the commissioner in the renegotiation of contracts, care and support plans.
1.5. In emergency situations, where an urgent response is required, work in cooperation with local safeguarding, crisis and inpatient teams to ensure risk and crisis management plans are implemented and service users are kept safe.
1.6. Establish and manage a risk and issues tracking mechanism for monitoring all placements and care packages. Put systems into place to ensure risks and issues are captured, with mitigating actions and solutions. Where required there should also be a defined governance and escalation route.
2. Information and Performance Monitoring
2.1. Work with the MH Information Analyst to devise and support improvements to current management information advising on required analysis and reporting and suggesting what information and data would be useful to enhance decision making and improvements to commissioning processes.
2.2. To record and report on baseline performance targets and engage in activity management plans and capacity reviews with commissioned providers.
2.3. Report key findings to commissioners to support service improvement and continued value-based commissioning.
3. Operational Responsibilities
3.1. Contribute to the strategic planning of Mental Health and Learning Disabilities projects, supporting commissioners to identify interdependencies across projects/programmes and how these will impact on each other.
3.2. Contribute to short, medium and long term business planning cycles, working with commissioners to identify areas which would benefit from development and improvement in order to achieve quality outcomes.
3.3. Keep informed and abreast of new technological advances in telecare and telehealth which could support more efficient and less intrusive methods of supporting self-care among service users with mental health and learning disabilities.
3.4. The post holder will be required to maintain constructive, professional relationships with a broad range of internal and external stakeholders. This includes (but is not limited to) local clinical teams, social care teams, inpatient ward staff and management, independent hospitals and community providers ICB and LA management, NHS England Specialist Commissioners.
3.5. Liaise closely with service users and their families and carers, to ensure their care promotes independence and tackles health inequalities personalized and tailored to meet their needs and that all individuals involved in the care management process are committed to this principle.
3.6. Work with the Information Analysts and Management Accountants to develop robust systems for monitoring placement / care spend against projected budget.
3.7. Present information and issues, which may be highly sensitive and emotive, to a wide range of internal and external stakeholders. This may be delivered in difficult and highly emotionally-charged situations, in which there may be disagreement between professionals.
3.8. Support in the ongoing development activities through participation in knowledge transfer and sharing initiatives and action learning sets.
3.9. Participate in policy and procedural development for commissioning and arranging specialist mental health and learning disability placements / care.
3.10. Maintain a good working knowledge base of the Mental Health Act provisions applicable to inpatient and community care, including statutory obligations for Aftercare service provision and the relationship to the Care Act and Responsible Commissioner guidelines.
3.11. Play a key role in contributing to the redesign of the existing service model in order to develop and procure services closer to home and in a manner which promotes optimum independence for clients.
3.12. Contribute to the review and development of existing information management systems and contribute to the development of an integrated approach to service or initiative management.
3.13. Work with ICB and Local Authority commissioners, and social workers to develop protocols for commissioning and managing jointly funded packages of care under S117.
