Band 7 Clinical Practitioner – Urgent Community Response Team

The Clinical Practitioner in Urgent Community Response will act as a generalist 'expert' and exercise their clinical expertise in long-term conditions (LTC's) including frailty, levels of judgement, discretion, and decision making in clinical care to deliver the Urgent Community Response Model.

Main duties of the job

  1. Work closely with the wider multi-disciplinary workforce in primary, community, and secondary care to ensure patients receive appropriate investigation, intervention, and treatment with minimal avoidable delays, whilst improving the quality of care in the right setting.
  2. Work autonomously assessing individual patient needs, initiating investigations, and developing a personalised clinical management plan, including independent prescribing.

Job responsibilities

  1. Be highly visible in the clinical area to enable a high quality responsive service including out of hours working.
  2. Be resilient and flexible to service demands to enable a high quality responsive enhanced care service, including out of hours working.
  3. Contribute and where appropriate lead the development of the Multi-Disciplinary Team (MDT) and team objectives for the area of expertise.
  4. Act as a role model of standards for behaviour and professions, through commitment to the integration of policies and procedures within role and workplace.
  5. Contribute to local networks and specialty specific groups, working in partnership with other key professionals.

Qualifications/Training

  • Registered first level nurse or equivalent professional registration e.g. HCPC
  • MSc or equivalent level knowledge/experience in relevant area
  • Clinical supervision training and experience
  • Extended/independent nurse or specialist

Experience

  • Recent primary and community/nursing/therapeutic experience
  • Nurse-led management of minor illness, minor ailments, and injuries
  • Nurse-led triage
  • Compiling protocols and clinical guidelines
  • Clinical Leadership
  • Audit
  • Research
  • Non-medical prescribing
  • Ability to work across boundaries

Skills/Knowledge

  • Management of patients with long-term conditions
