You will be responsible for identifying need and the best course of advice, support, or intervention, promoting psychosocial wellbeing by identifying and supporting pathways for service users and carers.

Crucial to this role will be your ability to ensure robust communication across mental health providers, VCSE organisations, and primary care in accordance with each individual service user's needs by providing support and advice to all colleagues within the PCN as required, including participation within the local neighbourhood MDTs.
