What you will do:

  1. Conduct comprehensive assessments of patients within their homes and care settings.
  2. Develop and implement tailored treatment plans to promote patient independence and quality of life.
  3. Prescribe and fit essential equipment, ensuring patient safety and comfort.
  4. Identify and address falls risks, delivering appropriate interventions to reduce occurrences.
  5. Collaborate with multidisciplinary teams to deliver holistic care solutions.
  6. Maintain accurate records and report on patient progress.
  7. Travel between patient homes and care homes in the Lewisham area, bringing necessary equipment.

To be successful in this role, you must have at least 1 year of experience working within the community, particularly with older adults. Experience in prescribing and fitting equipment, together with a strong understanding of falls risks and interventions, is essential. You will need to be a car owner and driver to efficiently navigate your duties, carrying equipment as necessary. A valid HCPC registration is also required.
