Job description

An exciting opportunity has arisen for a Band 6 Mental Health Practitioner to join our Neighbourhood Mental Health Team working within the Primary Care Networks (PCNs) in East Birmingham.

Within BSMHFT our community mental health services are well-established and as part of the BSOL community transformation programme, we are looking to increase our Neighbourhood Mental Health Teams with mental health practitioner roles. This is an exciting opportunity to be at the heart of national change and developments in the delivery of community mental health care. The development of our Neighbourhood mental health team is an essential service which we are in the process of evolving in line with the NHS Long Term Plan.

We are looking for enthusiastic and dynamic individuals who are able to work within a multi-disciplinary team, working alongside GP's and other primary care professionals who strive to achieve the best outcomes for individuals within the local community.

Main duties of the job

You will be responsible for providing mental health triage, assessment and outcomes-based intervention to meet service user's needs within the Neighbourhood Mental Health Team.

You will be responsible for identifying need and best course of advice, support or intervention, promoting psychosocial wellbeing by identifying and then supporting pathways for service users and carers.

Crucial to this role will be your ability to ensure robust communication across mental health providers, VCSE organisations and primary care in accordance with each individual service user's needs by providing support and advice to all colleagues with the PCN as required including participation within the local neighbourhood MDT's.

  • Relevant post registration qualification


Knowledge and Experience

  • Experience of leading or participating in service improvement initiatives.
