An established industry player is seeking a Band 6 Children's Community Learning Disability Nurse to join a dynamic and supportive team. We have an exciting opportunity for a Band 6 Children's Community Learning Disability Team to join our well-established dynamic, forward-thinking, and friendly Children's Community Learning Disability Team. A Band 5 development post will also be considered for the right applicant. The post is 30 hours per week; you will be required to work flexibly over 4 weekdays. You must be enthusiastic and motivated and have prior experience of working with children and young people with learning disabilities and challenging behaviour. Due to this being a community role, it is essential you have a full driving licence and access to a vehicle. In return, we can offer you clinical/peer supervision, support from the wider children's community nursing service, line management by the children's community LD team leader, and opportunities to develop professionally.
The post will require working with children and families on an individual basis, managing a caseload and delivering programmes of care. A range of community work will be undertaken, including specialist behavioural assessments to formulate interventions, strategies, and positive behaviour support plans. The post holder will contribute and facilitate parent training programmes such as "Riding the Rapids". The training of other professionals is also essential for this role. The post holder will be required to work primarily in the family home, but also in other settings such as school/hospital. Highly developed communication and problem-solving skills are necessary, along with resilience and emotional intelligence to work in emotionally challenging situations. The ability to prioritise workload and meet deadlines is essential. Good organisational skills, reporting skills, and writing outcome-focused care plans are required.
To provide a safe, responsive, efficient, timely, high-quality evidence-based nursing service to children and young people with learning disabilities, their families, and carers. To be aware of, and participate in meeting the psychological, social, physical, and spiritual needs of children and young people. Undertake a range of comprehensive assessments on children and young people with highly complex and challenging needs. Develop a range of person-centred interventions and specialised care plans. To coordinate, monitor, and evaluate highly complex interventions and packages of care. Respond appropriately and flexibly to unpredictable situations. To facilitate children and young people and their carers' understanding of highly complex issues in relation to their needs, considering significant communication impairments and their level of understanding. Work in collaboration with other agencies to promote, identify and facilitate the use of reasonable adjustments to ensure health needs are met and health outcomes are optimised. To embrace public health role, promoting health and wellbeing and where possible, prevent ill health. To undertake screening and triage assessments for newly referred children and young people. Identify, manage and minimise risk.
