Job description

This is a great development opportunity for any newly qualified or existing band 5 occupational therapist who wants to embed their core skills and gain experience in an intermediate care setting.

Intermediate care will provide the post holder with the opportunity to deliver high standard of rehabilitation through occupational therapy interventions to patients in intermediate care stepdown units in various bed-based units across Dudley Borough. The post holder will be responsible for assessing and treating patients with a wide range of clinical needs and conditions, including orthopaedic, neurological, medical and surgical, as well as frail elderly co-morbidities. The post holder is required to work as part of a team committed to delivering and improving high standards of therapy within Dudley group NHS Foundation Trust.

There will be opportunities to rotate into other community areas to gain additional skills and knowledge as the Community OT rotation develops.

Main duties of the job
  1. To provide occupational therapy assessment and treatment for patients in a community setting ensuring that a professional, high-quality service is delivered.
  2. To liaise with patient, families and carers and the multidisciplinary team.
  3. To liaise with colleagues in acute sector, social services and primary care, voluntary sector and other agencies.
  4. To establish good communications within the therapy service, members of the multi-disciplinary team, patients and visitors.
  5. To hold responsibility for own case load and be responsible for a defined area of the service, often working without direct supervision. Access to advice and support from the team member(s) is available as required. To undertake all aspects of clinical duties as an autonomous practitioner.

Job responsibilities

Clinical

  1. To be professionally and legally accountable for all aspects of your own work including the management of a caseload of patients in your care.
  2. To select and apply specialist OT assessment of patients for a designated caseload.
  3. To provide a lead role within the multi-agency team in facilitating and making recommendations regarding the safe management of patients within the community.
  4. To plan and implement client-centred individual interventions, using graded activity to achieve therapeutic goals.
  5. To monitor and evaluate treatment in order to measure progress and ensure effectiveness of intervention and alter treatment programmes as required.
  6. To assess the occupational needs of patients in the community and establish and evaluate appropriate treatment programmes.
  7. To advise on suitable equipment and adaptations, making recommendations and referrals onto other community services as necessary.
  8. To have the physical ability to carry out the necessary therapeutic handling and dexterity for assessment and manual treatment of patients.
  9. To take responsibility for the fitting and demonstration of the safe use of items of equipment for patients and family members.
  10. To impart complex information to patients, carers, and other members of the multidisciplinary team in a way that is informative and at a level that is understandable by the individual.
  11. To manage clinical risk within own caseload and have a working knowledge of clinical governance.
  12. To provide spontaneous and planned advice, teaching and instruction to patients, carers, relatives and other professionals to promote the understanding of occupational therapy.
  13. To represent the ICT service at multidisciplinary team meetings to ensure the delivery of a coordinated service.
  14. To provide support for therapy assistant staff working in the specified area.
  15. To delegate some relevant treatment and or assessments to therapy assistant members of staff, following completion of the competency framework.
Person Specification
Qualification & Education
  • Degree/Diploma/MSc Pre Reg in Occupational Therapy
  • If a student, must be in the final 6 months of course
  • Health Care Professional Council (HCPC) registration
Knowledge & Training
  • Evidence of theoretical knowledge and continuous professional development
  • Knowledge of the wider Multi-disciplinary team
  • Basic Knowledge of principles and practice of Clinical Governance and its application to Occupational Therapy
  • Knowledge of evidence-based practice its relevance to occupational therapy
  • Knowledge across the range of Occupational Therapy practices in community sectors
Experience
  • Relevant undergraduate experience or sound knowledge base and ability to adapt to community setting
  • Awareness of relevant national and local guidelines
  • Ability to demonstrate understanding of NHS and the challenges facing the Occupational therapy profession

  • Ability to build effective working relationships with colleagues, relatives and patients
  • Ability to communicate effectively with all members of the multi-disciplinary team, at all levels

  • Ability to analyse and make clinical judgments about assessments and plans with patients

  • Evidence of good time management and personal organisation skills
  • Must be willing to participate in flexible working patterns

  • Able to commute to meet the needs of the job - traveling between sites, being able to deliver equipment
